Neela is walking around in the ER naked and chatting with Sam and Morris about her rotation being over.  As they talk, we see different elements of her thoughts in her surroundings, such as models being photographed in an exam room after she mentions that she is going to catch of with repeats of "Project Runway" when the cable guy comes to fix her cable after it being out for three weeks.  Harold walks up and talks with her and then Neela walks outside singing a song.  She then looks up and it starts to rain.  We hear a voice calling for her and Neela wakes up as she is taking a shower in the locker room at the hospital.  Dr. Dubenko is the one calling her name.  He tells her that there was a mix-up and that her shift in General Surgery started fifteen minutes ago.  She is disappointed about it.

Gates and Chloe are spending time with Chloe's grandparents.  Tony and Becky Riley are discussing how well Chloe is doing as they watch Chloe and her grandfather play air hockey.  Chloe beats him and he is out of breath.  He does not look well.

Pratt is in a Management Seminar Workshop doing a role-playing exercise with two guys.  Pratt does not seem to be doing well trying to control the two in their argument.  We then see how Pratt handles Frank and Morris arguing about Morris eating a sandwich in the work area.  Frank takes the sandwich from Morris and throws it out.  Sam walks up and talks to Pratt about a patient named Mr. Yanai who was kicked out of his band because of his blood pressure medication is making him tone deaf.  Pratt has never heard of that.  Sam receives a bouquet of a dozen roses and Morris wants to know who they are from.  Sam will not tell him, she says that they are from a female patient that she had helped earlier.  Neela walks up and asks Frank what the date is.  He tells her April 17th and asks if she has done her taxes yet.  He then shows her a picture of Abby, Luka and Joe over in Croatia.  Frank tells her that she needs to get one of those and she asks, which a husband or a baby?  Morris and Harold walk up with a patient on a gurney named Sheryl Hawkins.  She had been in a car accident and the car rolled three times.  Harold is trying to talk Neela into eating in the cafeteria with him.

The paramedics have arrived and they are checking on Hank Riley.  He hit his head and has a laceration.  Tony tells him that he has to go to the hospital, but he does not want to.  He says it is because he skipped breakfast and it caused him to faint.  They discuss if he is going to go in or not.

Sam and Pratt walk outside as they are waiting on a paramedic run.  Sam asks him if he has ever sent a woman roses and they discuss it.  Pratt sees Bettina and goes to speak with her.  He tells her that he has been leaving a lot of messages, but never returns his calls.  She tells him that she has been busy.  He asks if she would like to go grab some coffee later, but she tells him she is sorry and has to go and then walks away.

Neela, Harold and Morris are working on Sheryl Hawkins.  She is really dizzy as she tells them that she is a freshman in college and has four kids.  As she is speaking she begins to faint.  Neela suspects that she may have something wrong with her aorta, but Morris thinks that Neela is hallucinating from the long hours that she has been working.  Harold follows Neela out of the exam room and mentions grabbing a coffee.  He leans over like he is trying to kiss her, but she pulls back.  He tells her that he was just trying to smell her breath to see what might be missing from her diet.  She walks away and tells him to quit hitting on her.  Sam gets her for a trauma consult.

The consult is on Sarah's grandfather, Hank.  Tony, Sarah and her grandmother are together.  Morris is checking him out.  Sarah sees Neela and Neela flashes back to the time when she was trying to save Sarah's mom's life after her overdose in Season 13's "Murmurs of the Heart".  During that scene, Sarah is mad at Neela for not doing anything to save her and Tony had quit performing CPR on her mom Meg.

Neela and Morris continue working on Hank as Tony, Sarah and Becky watch on.  Lily comes to get Neela for her other patient.  Neela tells Becky that the head trauma was minor and that the problem is cardiac.  Hank wants to get some medicine and go home, but Morris tells him that he needs to stay for a day or two to make sure he didn't have a heart attack.  Tony sees that Sarah is concerned and tells her that her grandfather will be okay.  Tony asks Sam if she received her special delivery and she tells him yes and thanks him.

Pratt is working on Mr. Senula, who seems to have some head trauma.  He has a fractured radius and ulna.  Sam points out from the x-ray that he has a Humeral head also.  It looks like he has been beaten several times during the past.  Frank hands Pratt a piece of paper for Pratt to sign.  He tells Dr. Pratt that he is filing a grievance on Dr. Morris and his aftershave.  Frank tells Pratt that Dr. Brenner's connecting flight had been cancelled and that he will not be in until tonight.  Sam goes out to talk to Mr. Senula's son Nick.  Pratt follows her.  They find out that it is only the two of them and an aunt, but the boy does not remember the aunt's number.  They tell Nick that his father is showing symptoms of violence, but Nick tells them that he does not know of anyone that is hurting his father.  Nick seems to be aggravated by their questioning and tells them that his father's arms are all busted and he doesn't know how it happened.  As he walks to go see his father, Sam and Pratt realize that they never mentioned that his arms had been hurt.

Hanks condition has worsen.  He has a lot of belly pain.  Morris works on him and Tony helps.  He is in a lot of pain.

Neela is wheeling Sheryl in to surgery.  Sheryl is on a cell phone speaking to her five-year-old daughter Lucy.  Sheryl thanks Neela for letting her speak with her.  Dubenko and Harold are scrubbing in and Dubenko tells Neela to scrub in and that she can be first assistant.  She tells him that she has been awake for like thirty hours.  "I'll let you to the aortic repair yourself.", he tells her.  Harold tells her that she has to do it.  It will be a great way to spend their last case together.  He is going to Pedes ICU, which surprises Neela, because she thought he would be going to Pedes surgery.  She tells him that he only has five years and needs to take advantage of it.  He does not need to be doing a non-surgical rotation she tells him.  "You need to make every month count".

Morris is still aggravated with Frank.  He thinks Frank is hiding his food from him.  He tapes Franks desk and posts signs that say "Frank Sucks".  Sam is talking with Nick about his dad and accuses him of hurting his dad.  He gets mad and storms off.  Pratt gets on to her and she talks back to him.

Pratt goes outside and speaks with Nick.  Nick tells him that his dad has been falling a lot lately.  He tells Pratt that his father was in a car accident a few years ago and that he ended up having to stay with a foster family.  He is the one that called 911, but he disguised his voice to make himself sound older.  There really isn't an aunt for him to stay with.  He was afraid that he would have to stay with a foster family again and did not want to have to go through that.  Pratt starts to think about why Nick's father is falling a lot and wants to go check some things out.

Morris brings Hank back down from radiology.  His condition has worsen and he has been tubed.  Beck Riley is concerned and wants to know what needs to be done.  Morris wants surgery, but Gates doesn't.  Mrs. Riley get upset with Tony and tells him to stop contradicting Morris.  He has a big clot and it needs to be broken up.  The blood flow needs to be restored to his bowels.  Tony wants to use a clot busting medication to unblock the clot without surgery.  Morris wants them to page Neela for a consult.

Dubenko, Neela and Harold wheel Sheryl out from surgery.  Dubenko walks away and Neela starts talking to Harold about leaving surgery.  Harold tells her that he does not want to be a surgeon and she thinks it is Dubenko trying to "squeeze him out".  He tells her that Dubenko has been very supported of his decision.  She still does not listen and continues to state that she will take care of it and she gets into the elevator to go down to the ER for a consult.

Sam and Tony speak about what is going on with Hank.  She tells him that he does not know how to talk with women, but she can teach him.  They go back into the exam room where Neela is speaking with Mrs. Riley.  She tells her that surgery is the best option, but Tony and Sam speak up about other options.   Morris and Neela say that they will lose valuable time if they try the other options and Tony disagrees.  Beck yells out for Tony to stop it!  He always has to be right, she tells him.

Nick walks up to Pratt and asks about his dad.  Pratt tells Nick that his dad is not getting enough calcium because of his hormones causing too much calcium in his blood.  It is making his bones brittle and causes them to break easily.  He will need to have surgery to correct this and will need to be in the hospital for about a week.  Nick is concerned about having to go to a foster family, but Pratt tells him that as long as he comes and visits his father everyday and checks in with him, that Nick can stay alone until his dad returns home.  Nick thanks him.

Pratt sees Gates and asks about Hank.  Tony tells him that it can be better.  Neela walks up and talks to Tony about Julia leaving.  Neela can't believe that he has moved on t Sam now.  She tells him that he needs to be more concerned about taking care of Sarah and that having another woman in his bed every week is not helping.

Neela gives Mrs. Riley the surgical consent forms.  Mrs. Riley is not sure what to do.  She reads the forms and sees what can happen to him if he has surgery.  She tells Tony that maybe he was right.  Maybe doing the TPA to unblock the clot will be better and then Hank won't need the surgery.  Tony tells her that Dr. Rasgotra is an excellent surgeon and that he thinks surgery is the best option.  Mrs. Riley nods her head in agreement.

Dubenko and Neela perform surgery on Hank as the Riley's and Tony watch on from the viewing room.  Neela is not doing so well.

Pratt and Nick walk as they wheel his dad up to his room.  The father seems to be happy that Pratt worked things out with his son.  Pratt and Sam speak about what happened and she is glad that it all worked out.  He tells her that he has been taking a conflict management course and she laughs and says that he has a long way to go.  Morris and Frank get into it again.  Frank calls Morris a "garden gnome".  Pratt tries to use some of his new methods.  Frank tells him that it is because Archie had a party and did not invite him.  Archie begins to speak with Frank and Frank tells him that he may not like sushi and karaoke, but he could have grabbed a burger before he came  and then watched him do karaoke.  They hug and make up as Pratt watches on in amazement that his methods worked.

Neela and Harold finally get to have a talk.  He tells her again that it was his decision not to continue in surgery.  He wants to have a life.  He does not want to end up being alone because he would never have time to form relationships if he continued the course of being a surgeon.  He tells her that it is not her, but it is him. and walks away.

Pratt has stepped out to get a coffee or something and sees Bettina sitting with a man talking.  Pratt walks over and Bettina introduces him to Grant Hayward from Oncology.  Pratt is trying to chime in to their conversation about what is going on at the hospital, but Bettina tells him that they are not talking about work, but about her.

Tony sees Sam walking out with her dozen roses.  She tells him that they are beautiful, but kind of embarrassing.  Morris walks up and tells Sam that he has figured it out.  She has been sending the flowers to herself.  She agrees and walks away and then Archie turns around and sees that she is with Gates.

Neela and Dubenko have a chat in the lounge.  Neela tells him that her college roommate is having another baby shower.  She has turned down dates so that she can sleep.  She tells him that she has been dumped by a nineteen-year-old intern.  He tells her to go home, she is tired.  She tells him that  today is her thirty-first birthday.  He apologizes for not realizing it.  He gives her an old muffin.  She tells him that she fell asleep in the shower this morning and realizes that she has been going on like things are going okay, when they are not.  He then walks her around the recovery rooms to show what a great surgeon she is.  If it were not for her skills, these people's lives would not be the same.  "You have a gift Neela.  The minute you start comparing yourself to others is the minute you forget that you're doing exactly what you are meant to do."  He tells her to go home, her shift ended fifteen minutes ago.  She walks away and Sarah comes up and thanks her.

Bettina and Pratt talk.  She tells Pratt that she has colon problems.  She has colon cancer and that is why she was speaking with the guy from oncology.  She tells him that he wasn't 100% committed to their relationship and that she ended the relationship because she did not think that he could handle what is going to happen with her.  She does not need his pity.  He doesn't want her to go through it alone.  It is not his problem.  "I'm letting you off the hook", she tells him.

Tony tells Mrs. Riley that Hank will be in the hospital for a week and that he and Sarah would like to have her stay with them.  She thanks him for what he has done for her husband and apologizes for how she has treated him.  She tells him that he has come a long way from the messed up vet coming from the war and dating her daughter.  She then thanks Dr. Rasgotra.  Tony and Neela talk and she apologizes for what she said about him dating Sam.  She is sleep deprived.  He tells her Happy Birthday.  She is surprised that he remembered.  Neela answers her cell phone.  It is the cable company telling her that she missed her appointment and that it will be three weeks before they can come out again.  "Look, I'm a doctor and I'm single.  All I have is "Project Runway".  "Hello? Hello?"

NBC Review

PULLING AN ALL-NIGHTER IN THE ER- AIDA TUTURRO ("The Sopranos") GUEST STARS-- Neela (Parminder Nagra) has a strange daydream during work after she pulls an all-night shift. Sarah’s (guest star Chloe Greenfield) grandfather (guest star Bill Bolender) is rushed to the ER after collapsing during a game of air hockey. Gates (John Stamos) tries to let Sam (Linda Cardellini) know that he is serious by sending her flowers. Neela makes a quick decision about a patient with torn aorta (guest star Aida Tuturro).
