Kerry arrives at the ER in all of the turmoil that has just happened.  She tells the officer there that the people in the van that had the shootout in the ER may have taken one of their nurses, Samantha Taggart.  An eye-witness confirms that he saw them take a young lady with brown hair.  Kerry tells the officer that they need to find her.  Lily comes running out and Kerry asks her if everyone is okay and wonders how she got through the ER.  Lily tells her that she came through Oncology.  Kerry tells her to call O.E.M.C. and put them on medical bypass, the ER is closed for internal disaster.  She also tells Lily to page Dr. Kovac.  Lily tells her that she will get right on it.  Kerry walks in to the ER and tells Chuny to check out the equipment to see if anything was damaged as she looks at all of the ruble from the gun fire.  Frank has arrived after hearing about what has happened and watches on as the doctors and nurses work on Jerry.  "Jerry is going to be okay", she tells him with a concerned tone in her voice.  Weaver and Chuny see a little boy crouched down looking very concerned and upset.  It is Timmy, the little boy that Jerry was looking after before he was shot.  Chuny tells Kerry that she will take him to daycare while they wait for his parents arrival.  Weaver asks Haleh how Jerry is doing and she tells her that they are waiting for Abby to come back after going for some thoraseal.  Haleh then asks Dr. Weaver if there is any word on Sam?

In the van, Steve tells his accomplice Rafe that they should have waited.  Rafe tells him that they waited long enough.  "We would have walked right out of there if you wouldn't have let her play doctor", he replies as he refers to Mary their other accomplice.  Steve asks Sam if Alex is okay and she tells him that his sugar level is peaking and that he needs some food.  Steve seems surprised and Sam replies, "Yeah he is diabetic, what did you forget?"  They are about ten minutes from the place where they are going to switch vehicles and then they will get Alex some food he tells her.

Kerry walks in to the trauma room where Morris and Ray are working on Jerry.  Morris tells Weaver that they are waiting for Abby to return with the second thoraseal.  Weaver tells them that she will go see what has happened with it.  As she walks in to the next room, she finds Abby collapsed on the floor and bleeding.  "Abby, Abby, open your eyes", she tells her as she checks for a pulse at her neck.  "I need some help now", she yells out to the others.  She then stands up and sees Luka tied to a gurney with tape on his wrists and feet and sees that he has been intubated.  Kerry looks on in horror.

"We're coming in right now", Pratt says as he hangs up the cell phone as he and Neela are walking through the cemetery where she has just had a service for Michael.  He tells her that Jerry took a couple of hits and that they are trying to stabilize him.  She asks about Abby and he tells her that they found her in a pool of blood.  He tells her that Frank wasn't too sure about anything else.  Pratt tells her that he will drop her off on his way in to the ER, but Neela tells him that she wants to go with him.  "County General as fast as you can please", she tells the driver.

Weaver and Chuny are working on Luka and she extubates him.  Kerry asks Luka what happened and he tells her, "that trainee girl stabbed me with Veck and Sam did the intubation, how's Abby?"  Kerry tells him that she needs to check on her and then she tells Chuny what Luka needs, but he wants to go with her to check on Abby.  She tells Luka that he needs to be monitored for an hour, but agrees to let him sit on a stool while they work on Abby.  Morris is doing an ultrasound on Abby and she is still out of it for the most part.  Morris tells Weaver the vitals and she says that it sounds good.  Abby comes to and Luka explains what is going on.  "Congratulations it's a boy", Luka tells her as they watch the ultrasound screen.  Kerry is concerned about the bleeding and tells Abby that she may need a c-section, but Abby quickly tells her that she does not need a c-section.  Abby tells them to call Dr. Colburn and Morris tells her that she has been paged.  Lily comes in to tell Dr. Weaver that someone from psych. is there to see her and Weaver tells her that she will be right there.  Abby tells Dr. Weaver to send her on up to O.B., but Morris still wants to do another exam.  "Dr. Weaver, send me up to O.B.", Abby tells her again.  Chuny gives Dr. Weaver Abby's vitals and Weaver agrees to send her up to O.B. for observation.  Ray comes in to the room and tells them that they are having a rough time getting an airway on Jerry.  Morris goes to assist him.  Abby tells Luka that he should help with Jerry's intubation, but Luka quickly tells her that he is going up with her.  She talks Luka in to helping Jerry and he tells her to page him if anything changes with her and the baby.

Frank is answering the phones and referring people to Mercy because they have the ER closed.  Dr. Nelson from psych. tells Dr. Weaver that the clinic is taking any walk-ins with stress related issues.  Kerry thinks that that is a good idea.  Pratt and Neela have arrived and are escorted in by someone guarding the area.  Kerry tells the guard that they are okay and to let them in.  Pratt quickly asks about Jerry's condition.  Neela asks about Abby and Kerry tells her that Abby is good and that they sent her up to O.B. just to be safe.  Chuny tells Abby that Dr. Coburn is on her way in right now.  Neela sees Abby and asks her how she is doing.  She tells Neela that she thinks she must have hit her belly when she fells, but she is fine now.  Neela tells her that she will go up with her, but Abby tells her no, that she will be okay.  "It will be fun, we can play who had the worst day", she tells Abby.  Abby assures her that she is fine and Neela tells her that she will come and check on her later.

The fugitives have arrived to swap vehicles and Alex is not feeling well.  Sam talks Steve into letting her get him some food, but he will not let Alex go with her.  Mary tells them to get her some chips and Rafe goes with Sam to the store.  "Come on, stand up straight; be a man", Steve tells a slouching Alex.  "You're not a kid anymore", he reminds him.  Steve tries to convince Alex that he is really going to like Canada.  "I like Chicago", Alex replies in a sarcastic tone.

Morris and Ray are having a hard time intubating Jerry.  Neela and Frank watch on through the window.  Luka tries to assist, but he is still having issues breathing himself.  Haleh announces that Jerry's pressure is down again and Frank and Neela step in to check on Jerry.  "He's okay Frank, can somebody call his mom?", Ray asks.  "I'll do that right now", Frank replies and steps out.  "She can meet us up in the O.R.", Morris yells to Frank as he is walking away.

Sam and Rafe are walking through a convenient store and Sam is looking at the clerk who is busy looking at a Maxim magazine with Eva Longoria on the cover.  "Hey, grab what you need", Rafe tells her.  Sam tries to shoplift some items in front of the store camera, but Rafe will not let her.  He tells her, "No stealing baby, these people work for a living", as he empties her pockets.

Morris finally gets Jerry intubated with Luka's assistance talking him through it.  Dubenko comes down to get Jerry for surgery.  "Everybody upstairs is doing everything they can to help you guys out", Dubenko tells Luka as he is leaving the room.  Luka gives Morris a thumb's up for the job he did.

Dr. Coburn is working on Abby and she tells her that the baby looks good and that she is definitely contracting.  Abby tells her that she doesn't feel anything.  The belly trauma put her into pre-term labor and that the head has dropped down and that there is some placental bleeding she tells her.  Dr. Coburn reminds Abby that she was once a nurse up in O.B. and that she knows the care that she needs to be given.  "Today you're the patient and not the doctor", Dr. Coburn tells her.  "I'm not having an abruption", Abby tells her firmly.  Dr. Coburn reminds Abby that the trauma has put her at risk.  Abby is concerned about having a c-section and Dr. Coburn asks Abby to let her know if she has any more pain as she walks out of the room.

Rafe and Sam exit the store and Rafe has Sam's arm clutched in his hand.  Mary asks Rafe if he got her chips and he tells her yeah.  Mary has just come from the restroom and Sam asks her if she can have the key and Mary replies, "I'm warning you, it is nasty".  Rafe doesn't want Sam to go to the restroom because they need to leave, but Mary tells him to let her go to the restroom, "they can't hold it in like you guys".  Sam writes a note on a paper towel and leaves it in the sink with her hospital badge ID.  The note says KIDNAP  LIC # W312P10.  As she is exiting the restroom, Rafe comes in and removes the note and flushes it down the toilet and puts her badge ID in his coat pocket.  As she turns around to leave, Rafe pulls her by the back of her hair and tells her, "Next time you try something like that, I'll take it out on your kid".   

Luka is now in the room with Abby, who is now having some pain.  Luka's face is bruised and battered from the attack, but his concern is over Abby and his unborn baby boy.  Dr. Coburn has returned and is still concerned that there might be an abruption.  She is concerned that if the placenta separates that the baby will be deprived of oxygen.  Luka sees that Abby is upset and takes her hand gently to calm her and to show his concern also.  Dr. Coburn orders some meds for Abby and the nurse goes to insert them into Abby's IV, but Abby quickly stops her and tells her "no, no, no, NO!".  The medicine is suppose to mature the baby's lungs, Dr. Coburn tells Abby.  "I have two and a half months left to go and this baby is not coming until then", Abby tells them.  "That's what we're all hoping for", Luka tells her.  Abby tells them that she can wait two more weeks and that the risk of prematurity goes way down after then.  She will do bed rest, use a bed pan and won't move.  "You know that I want you to have a healthy baby", Dr. Coburn tells her.  "Then why do you keep focusing on the worst case scenario", Abby asks in a sad and disappointed voice.  Luka quickly answer, "That's her job".  Abby slowly reaches over and lets the nurse give her the medicine to help mature the baby's lungs.  Abby looks as if she lost the battle, but knows that it is for the best of the baby.

Weaver, Frank and Chuny are walking through the ER to decide if it is okay to re-open.  Chuny tells her that eight monitor beds are good to go.  Frank tells her that all of the night staff are coming in except for two.  Weaver then asks Frank if he has heard anything about Sam.  "A guy at the district called, they still have no leads", he tells her.  Lily tells Weaver that the E.M.T.s are wondering what the status is and she tells her to tell them that they can take 4 critical paramedic runs and then back on bypass until they see how they are doing.  An officer comes in and tells them that there are people showing up outside waiting to be seen.  Weaver tells Chuny to setup Fast Track in the west lobby and that they will speak to anyone who doesn't need x-rays or labs.  Pratt runs to assist.

The fugitives and their hostages are stuck in a line of traffic at a rail-road crossing.  Rafe decides that he can make it, so he crosses the track.  Sam tells him that he is going to get them all killed.  He replies, "The train is probably a mile away" and he crosses the track.  A police is in a line of traffic on the other side and spins around to pull them over.  Steve gets his gun ready and tells everybody to be cool and that nobody will get hurt.  When the officer comes to the window he asks for license and proof of insurance.  Steve tells them that they are heading to the lake so that his son can get some fishing in.  "It's his birthday", he tells the officer.  The officer tells them that he will be right back and heads back to his patrol car.  Steve feels assure that all is okay and that he is just going to give them a ticket.  Rafe replies, "He's a local yokel man, this is the most excitement he has seen all day".  Sam tells them that he is probably calling for backup.  "Every cop within a thousand miles radius knows what you guys look like", she tells them.  Rafe puts the vehicle in gear and flees the scene.

Jerry's mom has arrived and Morris is walking her to the waiting area.  She complains about the ER and how Jerry insists on working there.  Morris explains Jerry's condition to her and she wants to complain about the choices Jerry has made.  She applied to Harvard four times for him, but he was denied each time.  She then starts complaining about how they are going to pay the medical bills with the paycheck that Jerry receives.  Morris tells her that he will talk with her more later, but wants to go check in on Jerry during the surgery.  "He doesn't have any savings you know, but why would he?  He can barely survive off that weekly insult you people call a paycheck", she yells out as Morris is leaving the room.  Neela is in the observation room watching Jerry's surgery and Morris asks her how he is doing.  She tells him that a bullet nicked his heart.  Morris says, "A cardiac defect isn't so bad, it is repairable right?  It's not like he needs a new manectomy", he asks Neela knowing how knowledgeable she has become.  Dr. Dubenko is operating and Jerry goes into v-tach.  Dr. Dubenko gets the internal paddles, but it is not doing any good.  "He's having a heart attack:, Morris says out loud as they watch.  Neela calls in to the O.R. and interrupts Dr. Dubenko.  She suggests that the suture could be tying off the coronary artery.  This could be causing an MI she tells him.  "You can see inside the field", Morris asks her.  "I just had a hunch", she whispers back at him.  Dubenko does what Neela suggested and charges the paddles and get a sinus rhythm.  Dr. Dubenko asks Neela to scrub in and assist.

Rafe is weaving through traffic missing trucks and tractors as the police officer is chasing them.  "Let us out please, you don't need us anymore", Sam cries out.  "You're not prisoners you're my family", Steve yells back.  "You're going to get your family killed, is that what you want?", she tells him.  The officer has caught up with them and pulls up beside them.  Rafe starts to hit the officers car and knocks it into a construction barrier.  The cop car runs up over a ramp and flips over on top of a guard rail.  Sam again tries to get them to let them out and she suggests that she can go back and check on the injured officer.  Steve tells them all that no one is leaving.

Abby is concerned about how Luka is doing.  She tells him that Sam probably saved his life.  "I hope she's alright", he replies.  Luka is looking at the monitor and Abby asks how "he" (the baby) is doing?  He tells her to look for herself, but she replies that "the magnesium has drained all my life force".  "We're so not ready for this", she tells him in exasperation.  He tells her that no one is ever ready for this.  She is upset that they don't even have a car seat or a stroller, but he quickly reminds her that they have a crib.  "We don't have a name", she replies.  Luka tells her that he was kind of hoping that they could name him after his father.  He tells her that his father would love to have a grandson named "Mongo".  She looks a little concerned and he tells her that it is a very popular name in Croatia.  Luka starts laughing and tells her that he is just kidding, that his father's name is Joseph.  Abby seems to be relieved and tells him that it was not funny.  The laughing quickly turns to pain when Abby has a sudden burst of pain.  She tells Luka that she thinks her water just broke, but when Luka checks he sees a lot of blood.  Luka takes her IV and opens up the saline.  He then goes to the door to call for Dr. Coburn.  The baby's heart rate is dropping.  Dr. Coburn is ready to take them up to the O.R. because it seems to be an abruption.  Abby tells her that she wants a spinal, but Dr. Coburn tells her that general anesthesia is quicker.  Abby leans over and tells Luka that she wants to be awake.  Luka calmly tells her that she is going to be okay.

Sam and Alex are watching as Steve, Mary and Rafe are discussing what their next plans are.  Sam can see that they are disagreeing about what to do next.  "They know that they are in trouble and that the cops are coming to find us", she explains to Alex.  "Mom, is dad going to hurt us?", Alex asks.  She comforts him in her arms and says, "Everything is going to be okay buddy".  Steve, Mary and Rafe go inside an abandoned shed to discuss their plans further.  Sam sees an opportunity to rummage through their things to find the keys or a gun.  Alex tells her that he is scared and she replies, "I know your scared and I know I messed up a lot, but I would never, ever let anybody hurt you no matter what.  I swear to God."  Just as she finishes her sentence they hear three gun shots.  Sam and Alex run back toward the vehicle and watch as Steve exits the building by himself.  Steve puts the gun in the back of his pants and picks up some food in cartons and asks, "Hey, you guys hungry?"

Pratt walks in to find Frank sitting alone and thinking.  He asks Frank what is up and Frank replies, "You know I always believed that nothing never really changed.  I'm old enough that I know that there is always going to be trouble.  War, taxes bad guys now are pretty much the same as bad guys have always been.  That's not how I feel today.  I feel like the world's getting worse and worse and worse and that we're watching this slow steady decent of the human race".  Pratt replies, "No, no I don't buy that.  I think that today was just a rough day for the home team".

Neela scrubs in for Jerry's surgery as Dr. Coburn is scrubbing in for Abby's c-section.  Morris and Pratt watch on as Neela and Dr. Dubenko work on Jerry.  Luka is holding Abby's hand as Dr. Coburn works on her.  There is a lot of oozing as they cut open Abby's belly.  Their baby boy is removed and is non responsive.  Luka sees that Abby is losing a lot of blood.  Abby and Luka look on as the doctors work on their baby boy.  Neela and Dr. Dubenko have done a successful job with Jerry and Morris and Pratt are relieved as they watch on.  Dr Coburn is concerned because Abby's uterus is not responding to massage.  Abby is not clotting at all and she asks, "Am I in D.I.C."?  Abby's pressure is dropping.  Abby wants Luka to go with the bay to the N.I.C.U., but Luka feels like he should stay with Abby.  Abby tells him, "Shut up, shut up, shut up, I want you to stay with the baby.  I don't want him to be alone".  Dr. Coburn tells Luka that she will call him if they need to.  After Luka has left, Abby starts to cry.

Steve leaves Alex sleeping in the vehicle and joins Sam over on the ground where she has been sitting.  He talks about the stars and says, "We'll get him his insulin way before he needs it", he tells her, "he'll be fine".  Sam tells Steve that she was wondering how he became the way that he has, but soon realized that he was always going to end up this way from the moment that she met him.  Steve tells her that he loves Alex and knows that she don't believe that, "especially right now".  Sam replies, "Yeah you love him, right.  That's why you get some woman to snatch him from school and tie him up and leave him in a van.  You don't even remember his medicine".  Steve agrees that she is right and that it was his fault.  "It's my fault for letting us get separated in the first place", he tells her.  "But now we got to stay together, the three of us.  There's no other way.  I'd rather we were all dead", he tells her as he kisses her on the cheek.  Steve moves in front of her tells her that he doesn't want her to be afraid of him.  He proceeds to lay her down and rape her as she watches Alex sleeping in the vehicle.  "As long as we are together you and Alex are safe".

Dr. Coburn is still trying to control Abby's bleeding.  Dr. Coburn tells her that they can wait, but they need to start making a decision about the possibility of a hysterectomy.  Abby asks how long they can wait and Dr. Coburn tells her that they can only transfuse for so long before she goes back in to D.I.C..  Luka is in the N.I.C.U. with his baby boy looking down at him in an incubator.  Kerry comes in to see how they are doing.  She tells him that Jerry is in recovery.  He asks her if they have heard anything about Sam and she tells him no.  "This is a scary place.  I remember the feeling just walking in", she tells him.  (She is referring to the time that she was there with her and Sandy's baby boy).  "I know your baby is a lot sicker than Henry was, but everything tuned out okay for my boy Luka and I know that it will for yours too".  Luka tells her that they are concerned about different things and he begins to break down in front of her.  He regains his composure and continues talking to her.  "There's nothing more that you can do for him right now; you should go be with Abby", she tells him.  "She doesn't want him to be alone", he replies.  She tells him, "He won't be".

Pratt, Neela and Morris are visiting Jerry in his room and they tell them about his operation.  Jerry's mom comes in and she tells him how concerned she was about him.  Neela and Pratt leave the room and Morris gets stuck with her once again, but this time he has Jerry by his side.  Luka comes to check on Abby who is sitting up in bed.  She asks about the baby and he tells her that he is doing fine.  It is good news to her.  "Is one going to be enough?", she asks him.  She tells him that they could not stop the uterus from bleeding and that Dr. Coburn did all she could.  She had a hysterectomy.  Luka tells her that one is all they need.  Abby tells him that she won't be able to get to the N.I.C.U. for another twenty-four hours and Luka pulls out a handheld device.  He has a video for Abby to be able to watch her baby boy in the N.I.C.U. as he moves around and waves his arm.  "That's what I told him, to wave to mom", Luka says as he caresses Abby and kisses her hand.  "He's a smart one", he exclaims.  Abby tells Luka that she wants to name him Joseph and tells a story about one of the few memories of her dad.  He liked watching boxing and especially Joe Frazier, because he was a pure fighter.  She wants to call him Joe, if that is okay.  Luka stands and kisses her.  He tells her that he will be right back and he heads to the restroom.  He washes his hands thoroughly and then starts to cry.  He picks up the trash can and throws it into the mirror which causes it to shatter.

Sam is awake and Steve is sleeping next to her.  She says his name a few times and then moves his arm off of her and gets up.  She runs to the vehicle and sees that Alex is still sleeping.  She has the keys, but gets back out of the vehicle.  She takes the gun from Steve while he is sleeping and aims it at him.  We hear three shots.

NBC Review

A SHOOTOUT LEAVES THE 'ER' IN CRITICAL CONDITION-- As the 'ER' recovers from the shootout, Steve (Garret Dillahunt), Mary (Natasha Gregson Wagner) and Rafe (Michael Weston) make their way to Canada with Sam (Linda Cardellini) and Alex (Dominic Janes). Steve resorts to desperate measures when his partners in crime suggest that Sam and Alex are hindering their escape. Meanwhile, Weaver (Laura Innes) finds Abby (Maura Tierney) passed out and bleeding and rushes to make sure the baby is safe. Dr. Dubenko (Leland Orser) and Neela (Parminder Nagra) struggle to stabilize Jerry's injuries. Goran Visnjic, Mekhi Phifer, Shane West and Scott Grimes also star.

As the 13th season opens, Weaver, Kovac and the other doctors work frantically to try to save Jerry and a pregnant Abby, who were critically injured during the shootout that left the ER in shambles.
