A man in an electric wheel chair goes out onto a crowded ice skating rink and rides around. He is brought to the ER, Ray and Haleh take care of him. His name is Nate Lennox and he keeps trying to leave. He uses his eyes to make the computer talk. "I have Amyotrophic Lateral Sclerosis also known as Lou Gehrig’s disease, It’s a progressive neurological disorder that has weakened my muscles, all of my senses are intact, my mind is clear" Sam says that he looks familiar and she may have treated him a few years back.

Flash back, 2004. He is speaking to a class and begins to experience difficulty breathing as well as speaking, he is brought to County where Sam and Neela treated him. His assistant, Fran, is there with him. In the hospital he refuses to use the communication aid and shoves the computer onto the floor.

Back to Today. Fran arrives and discovers that Nate has pneumonia, they ask him why he left home and his computer says "Free".

Flash back, 2002. Nate begins to choke on a piece of food while Dr.Anspaugh is giving the teacher of the year award. Nate is brought to the ER again. Pratt is his doctor and he inserts a feeding tube because Nate’s facial muscles and throat muscles have deteriorated.

Back to Today. Abby runs a test and finds that his lung capacity has gone down to 30 percent. Abby wants do a trake but Fran disagrees, They ask Nate and he says "Fine" "I'm fine" this causes further problems but they put the trake in anyway. Nate is set back more from the blood loss but recovers again. It is obvious that he is in pain so they order 4 of morphine but they remember that he had a bad reaction to the morphine before.

Flash back, 2001. Nate falls down a flight of stairs and breaks his wrist, he ends up in the hospital again. The doctors and Fran suggest that he used a wheelchair but he dose not want to. He tells Luka how Fran had worked for him for 15 years, she was right out of college. He reacted to the morphine and threw up.

Back to Today. Abby and Fran disagrees about where he should be, Abby wants him to stay in the hospital for a few days for observation, Fran says that she can take care of him and wants to bring him home that day.

Flash back, 1999. Nate tells Fran that he was diagnosed with ALS.

Back to Today. Nate wakes up and asks about his throat. He says he dose not want the trake and he dose not want to continue. Once he loses his eye movement he cant even communicate. He leaves but Abby chases after him, she tells him that he can fight the disease but he says that he is to tired. Abby bets him 10 bucks that he will pull through, all he has to do is go back into the hospital and he agrees.

Flash back, 1999. Nate is teaching a class about ATP. This is Abby’s class. At the end of class she goes up to him with a paper, she wants to drop the class because she is having trouble memorizing everything, he dose not sign the paper and he bets her 10 bucks that she can pass the class. As he is leaving he tells her when his office hours are as an offer to tutor her and she accepts.

Back to Today. Abby goes ice skating on the same rink that Nate was on earlier that day.

NBC Review

A LONG GOODBYE- JAMES WOODS AND ALLY WALKER GUEST STAR-- When a wandering ALS (Lou Gehrig's disease) patient arrives at the ER, many of the doctors are shocked when they recognize the former medical school professor (Guest star James Woods). Abby (Maura Tierney) and Kovac (Goran Visnjic) try to understand Dr. Lennox's wishes for his life after the professor is struck with pneumonia as well as the nerve degenerating disease. As his condition deteriorates, Abby struggles with the possibility of losing her beloved professor. After a controversial decision to perform a trake procedure to help Dr. Lennox breathe, Abby and Fran (Guest star Ally Walker) wonder if they made the right choice. Mehki Phifer, Parminder Nagra, and Linda Cardellini also star.
