The episode starts off with a man, Vincent Jansen (age 34), and young girl, Sydney Carlisle (age 10), eating breakfast at a diner. The little girl is picky and listless, Vincent asks for the check but is told that their cash register is broken but it’ll be fixed soon. He grabs the girl and heads out of the restaurant through the kitchen and out the back door. Police cars pull up on them and the man picks the little girl up, pulls out a gun and starts shooting wildly. A shoot out happens between the man and the police, ending when Vincent takes a bullet to the forehead. Upon approach it is discovered that Sydney was shot in the stomach. 

In the ER, Eve and Dr. Clemente demonstrate the latest equipment to the staff and no one takes interest. Abby and Luka are called on a case and go outside to wait on the rig. Once outside in the ambulance bay, Luka rages about how Clemente suspended Abby and Neela for “the monkey business.”  Abby shrugs it off as the ambulance pulls up. The patient is the young girl from the diner. It is found out that she wasn’t really the man’s daughter, he kidnapped her.

Moments later a young police officer from the shooting is brought in, and is taken care of by Sam. As she brings his chart to Admit, Frank harasses her about Haleh and asks who she plans on firing next. Pratt, Luka and Ray are now taking care of the police officer also.

Abby asks Sydney if she was molested.

“Only when we play the married game, Vincent loves me,” the little girl replies sadly. Chuny does a rape kit and it is determined that she was raped.

The man from the diner (Vincent,) is brought in next, Clemente butts in but is kicked out within the minute he enters the trauma room. Eve is now helping with the police officer, keeper him amazingly calm by asking questions about the police force. Over in Sydney’s room, she’s asking about her stuffed animal puppy that was lost at the crime scene. Abby calls her mother and reassures her about her daughter before handing the phone over to Sydney who immediately starts to sob at the sound of her mother’s voice. With the conversation ended, Dubanko and Clemente come check on the girl and disagree on treatment.

Neela and Luka are working on Vincent now; they talk about Michael’s homecoming that night when suddenly, Vincent wakes up in a panic. He grabs Neela and almost crushes her hand. They calm him down and he decides he wants to see Sydney, once again becoming hysterical.

“You want me to die!” he screams when Neela refuses.

“I want you to shut up!” the frustrated Neela mutters back. Vincent needs surgery soon, but there isn’t any operating rooms available at the moment. Dubanko comes in and asks Neela assist him on an exploratory surgery.

Back in the trauma room with Abby and Sydney, Sydney has lost consciousness.

“Sydney, come on honey, come on baby,” Abby pleads before rushing off to get help.

Eve is talking to the police officers that are waiting, she walks off with Sam and confides to her that she once had a fiancée who was a cop, he died on the job. With this knowledge and newfound kinship, Sam explains her situation with Steve (Alex’s father,) and asks her to look into why he might be coming to County. Eve agrees and the pair part.

Zoe (from the previous episode,) and her father come into the ER. He wants to know why she was given a prescription and what it was for. Neela tells him to wait, or else she’ll have Jerry make him wait. After Zoe and her father go back to chairs, Jerry tells Neela that he doesn’t like violence.

“I know, but he doesn’t,” she replies.

Neela returns to the curtain area where Vincent is and starts the surgery. In the middle of it, Jerry comes in saying Neela has a long-distance phone call. She tells him to get the number and she’ll call later.

Sydney still has a tacky rhythm; Luka and Clemente get angry at each other as they treat her. Luka gets so angry he stops working on her, while Abby tries to keep it together and help Clemente. The two male doctors start to bicker again when Abby shouts at them to “just stop!” Finally they call a time of death. Abby sadly sews the ten year old up.

Neela has finished working on Vincent, and is being harassed by a paramedic. He offers hockey tickets or a quiet night in. Neela shrugs him off and attempts to call the number she was given by Jerry. But is harshly interrupted by Zoe’s father… again. She makes up some lies about confidentiality, when Ray walks by. Neela excuses herself and pulls Ray aside and bluntly tells him he could go to jail if he tries to tell them. Neela wanders out into the ambulance bay and dials the number. It’s Michael’s superior, saying something about Michael not being able to come home. Because she is on her cell phone, Neela can get the details but is finally reassured that Michael is fine, he is just needed as a doctor some more because of a patient overload.

Luka and Clemente are still arguing about Sydney’s treatment, clashing over styles and letting things get out of hand, when Clemente has a nosebleed. He hurries into the lounge where Abby finds him and takes care of him. Clemente awkwardly apologizes, and Abby forgives him bluntly.

Luka is talking to Weaver about being Chief, and at first she doesn’t take him seriously but when he persists, she agrees to submit his name to the committee.

Ray and Neela are off at the same time, as they head out in the ambulance bay (after skipping the mandatory meeting,) someone jumps Ray. Tackling him and beating him wildly. It turns out to Zoe’s father, Neela is hysterical and Ray doesn’t fight back. Finally the man has finished and Neela tends to Ray.

Luka is home (all alone,) when the doorbell rings. Abby is turning to leave, he calls out and she shuffles in.

“Are you mad about something?” Luka questions softly.

Abby erupts immediately, splitting to ranting about Luka and Clemente being professionals and sobbing. Without answering her speech, Luka grasps her face and gives her a soft kiss.

EMOTIONS RUN HIGH WHEN A DIFFERENCE OF OPINION MEANS LIFE OR DEATH-JOHN STAMOS GUEST STARS--Neela (Parminder Nagra) gets all dolled up for Gallant's homecoming (Sharif Atkins) and her co-workers seem to take notice but, her plans change when she receives a call from Iraq. When a cop and a little girl become victims of a shoot out, Neela is plagued with the decision of whether or not she should save the shooters life.
