Bulldozers are going as Carter and Abby are at the sight of the Joshua Carter Center.  Carter looks at the dirt and wonders what do with it.  Abby asks Carter when it opens.  “A year from September.” he tells her and then congratulates him for making tenure.  Carter doesn’t seem very happy about that.  He asks if she is ready to be an R-2 because she has two weeks left on her internship.  Her reply is, “Do I have a choice.”  She then asks who Joshua is, and he tells her that Joshua was what Kem and he was going to name their baby.   They leave and walk to work together.  At the ER on Monday morning, Susan is with Morris going over things that are expected of his as Chief Resident.  Residents will now be going to him and any mistakes they make will be a reflection of his leadership skills.  Susan tells him that he will have to put in office hours now and all he hears is office and wants to know where it is.  She explains that it’s only a figure of speech.  She then a contract for them to sign for the next years residency and tells Ray that they need to talk about his contract later.  Luka and Sam are at a couple’s counselor.  The counselor asks basic background information about them, and when she gets to why they’re here, she replies, “I don’t think Luka and I should be together.

 

In a trauma, Pratt, Neela, and Chuny are working on a guy that won’t sit still.  Carter comes in and asks what happened. Neela tells him that Mr. Lillion was checking radiator leaks with the engine on when he caught his hair in a fan belt, and it torn out his hair when he tried to pull it out of the fan and cut his hand open.  Pratt asks Neela if she wants antibiotics.  She says a gram of ancef IV piggyback.  Pratt asks, “Anything else?”  She pauses.  Pratt jump up and says, “100 Gentemician. Why?”  She again pauses.  Then Carter tells her open fractures are susceptible to gram negative organisms.  Frank come in and tells Pratt that Darnell is outside.  Carter tells him to go and Pratt leaves.  He shakes hands with Darnell, meets a kid named K.C., and Darnell wants to know if there is a place they can talk in privately.  Luka and Sam are now talking about each other in counseling.  Sam tells her that she doesn’t think Luka is happy.  That he is closed off and never talks or shows his emotions.  He denies this, gets mad, and tells Sam that he guessed he should break a dish to show his emotions.  Then the counselor asks Luka what he is thinking.  He tells her that he is happy and Sam is not, they work together, and they have a good like.  This makes Sam want to hit Luka in the head with a hammer. The counselor asks what made them come today.  Sam tells her that she might have been pregnant and he didn’t seem to care.  In the ER, sewage is falling to a patient, literally.  Abby and Neela move the man and she tells Frank to call housekeeping about the leak.  Frank asks where Carter is and is told that he took the lost hair victim up to the OR.  Frank then tells Kem to leave a message for Carter. Susan walks up and wants to talk to Ray.  Darnell explains to Pratt that K.J. got busted by the police and wants him to do his community service hours at County.  It’s not his decision, but he will ask.  Sam and Luka are now talking about the no pregnancy.  Sam doesn’t think it would be fair to Alex, but Luka thinks he would like a little brother or sister to play with.  Sam then tells him that she didn’t even want a kid the first time, and after what happened to his wife and kids, does he really want to bring another kid into the world.  He wants to know how that is relevant to the situation.  She replies, “It is a huge part of what you are, and you won’t let me in.”  At the ER, Abby asks Carter how names like: Destiny, Miracle, or Faith ends up with leukemia.  Frank then tells Carter that Kem called and she sounded upset.  Maintenance places a bucket under the sewage to collect it, and everyone wants to know how long they will have raw sewage leaking from the ceiling.  Abby gets mad and asks what happens when the bucket fills up.  Carter is on the phone as Morris tells Frank to move boxes out of the Chief Residence office.  All of a sudden Carter gets serious and asks, “What happened.”  Susan and Ray are talking in a room.  She tells him that his was lucky to get into County and all he seems to clock, but has the ability to be a great doctor.  She then tells him not to sign the contract unless he is 100% sure of what he wants to do.  Carter is getting his things together and Abby walks in wondering if something happened to Kem.  He tells her that it is her mother in Paris.  He walks out telling Frank that he has to go and will call Susan from O’Hare.  Sam and Luka are leaving the counselor and he asks if she needs anything from the bank or Home Depot.  She gives him the look and walks off. 

 

Tuesday, Carter is riding in a car going to the hospital where Kem’s mom is.  Susan tells Luka that Carter went to Paris.  Morris gives Abby and Neela schedules that are not fair, but Ray gets a better one because he and Morris are going to a Cubs game.  Abby accuses Morris of taking bribes.  He tells her that would be unethical, but would like U2 tickets.  Abby and Neela complain to Susan, and she tells them that he is the chief.  Luka asks Susan if they can make Abby the chief, but she tells him to give it time.  Sam walks right past Luka and smarts off.  Jerry tells him that flowers usually work.  Pratt introduces K.J. to Jerry.  Pratt tells K.J that he will be running stuff up to the lab and if he doesn’t like it he can leave.  A septic newborn trauma pulls up and Pratt and Neela take it.  Carter arrives at the hospital and finds Kem’s mom sleeping.  As he leaves the room, Kem sees him and they hug.  Kem tells him that he should have told her that he was coming.  He tried to call her cell phone, but it doesn’t work in Paris.  He finds out that she is fine and meets one of Kem’s friends, Michel Timbaud.  Neela and Pratt work on the infant and put I.V. fluids and antibiotics.  Pratt leaves to check on another patient.  The mother then tells Neela that she is hurting her.  She then tells her that her baby has an infection and they need to give her antibiotics to stop it.  She also tells Chuny to put in umbilical lines.  Pratt tells Jerry to call the NICU on the newborn and Jerry tells him that K.C. doesn’t seem too happy and asks Pratt why is he here if he doesn’t want to work.  He says murder and Morris gets scared.  Then he tells Jerry that he tried to spray paint and cop car, with the cop still inside.  Abby ask Pratt if she can get a C.T. and Morris nods his head.  Morris then asks Pratt if the interns will be ready to supervise in two weeks and Pratt says no way.  Carter is leaving the hospital and Kem stops him.  He tells her that should have called her.  Kem tells him that Michel is just a friend.  Carter almost heartbroken asks if they are a couple.  She tells him in the past and he admits he is jealous.  He then asks some personal questions and starts to leave.  Kem then talks him into staying for the night.  Neela is still working on the newborn; Pratt comes back and finds out that she put in umbilical lines.  He gets mad at her and scares mother half to death.  Michel drops Carter off at the hotel and Kem tells him that she will come by in the morning for breakfast.  Carter still jealous asks if Michel will be there and is relived that he is not.  Pratt chews Neela out for putting in umbilical line in the infant.  He explains to her that she can’t go play NICU doctor whenever she feels like it and was lucky this time.  Neela starts to cry and leaves.

 

Carter gets a phone call in the middle of the night and meets Kem outside where she wants to show him something.  They run to the subway and he asks where they are going.  She tells him to have patience for once in his life.  She makes up stories of the people on the train as people gather. When the train goes outside, the sun is rising and she points showing him the Eiffel Tower.  It is now Wednesday morning in the ER and Neela is outside in the ambulance bay deciding to call in sick or not because Pratt is on.  Jake shows up and, Abby tells him what happened yesterday, and then leaves.  Abby tells her to get back on that horse and they walk in.  Morris wants Susan to have someone paint his office. She gets mad at him, walks off, and tells Pratt to take the stabbing victim in Trauma 1.  When he gets in there, Ray and Neela are working on it and do a good job of running the code.  Pratt tells them that there maybe hope in them after all.  Carter and Kem are having breakfast and tell Kem that Susan didn’t get tenure and she is mad at him.  Kem eats a donut and gets jam on her face, Carter wipes it off.  Jake and Abby are walking in the ER, and he asks for advice on ranking where he would want to do his residency, then a patient tells them that sewage is again coming from the ceiling.  She tells Susan who tells Jerry to close to trauma until the problem is fixed.  Abby takes a man who is have trouble breathing and tells Jake to hook him up to a monitor.  She then sees Birdie and Rebecca and goes to check on them.  Abby find out that Rebecca stabbed their niece, Shelley, because she was trying to strangle Birdie.  Carter and Kem walk in the park as Kem tells him about her childhood.  Carter then tells her that their son would be a year old now.  She starts to walk away and he grabs her.  He tells her that they has a huge tragedy, he loves her and would give up everything to be with her.  She goes back to the hospital and asks him to come see her mom before he leaves.

 

Ray, Neela, and Pratt are working on the niece, Shelley.  Abby comes in and tells Ray that his former patient, Rebecca stabbed her.  Abby tells him he should have ordered a follow-up.  He tells her that he did, but can’t follow them around 24/7.  As Shelley goes into shock, Ray asks anyone if she has heart problems.  Abby tells him that he is the only one who has ever met her.  Michel and Kem are talking to Kem’s mom in French.  She looks at Carter and stops because it is rude.  She sends Michel and Kem to get her something to eat so that she can talk to Carter alone.  She tells him not to give up on her.  Kem hears this and tells her mom to leave him alone.  Still working on Shelley, Pratt does a crack down and when he cracks the chest no blood comes out.  He tries a needle but all he gets is air.  Ray then discovers that he removed the dressing before the intubations and all of the air got sucked in her jugular causing an air embolisms, she dies.  Jake gets Abby and tells her that the emphysema patient is coding.  Abby puts in a breathing tube and Pratt helps her.  Neela tells Birdie and Rebecca that Shelley is dead.  Pratt shocks the patient and gets him back.  Susan comes over to help and tells Abby that she failed to recall basic respiratory principals, and by giving him nebs and oxygen it sent him into v-tack.  Neela makes Ray tell Birdie and Rebecca that the stab wound didn’t kill their niece.  Carter is leaving in a taxi and tells the driver to return from where they came from.  Jake walks outside and tells Abby that he is going to rank County as his #1.  She says not to do that just because of her.  Neela and Ray walk out and have found out that Shelley was using them to support her Coke habit.  She would beat them if they didn’t bring home enough money.  Then Morris comes out prissing his ass, makes fun of all their mistakes and tells Pratt that they will never be ready to supervise the new interns in two weeks.  Then everybody leaves but Abby who stays outside on the bench.  Carter is setting in traffic and he can’t wait anymore.  So he pays   the driver and starts to run in the rain.  He get to Kem’s apartment, runs up the stairs, and he tells her that he is moving back to Africa and he love her.

CARTER RE-UNITES WITH KEM IN PARIS WHILE EVERYTHING GOES WRONG IN THE ER -- THANDIE NEWTON GUEST STARS -- Pratt (Mekhi Phifer) explodes at Neela (Parminder Nagra) for performing a complicated procedure on a baby, Abby (Maura Tierney) misdiagnoses an emphysema patient, and Ray's (Shane West) procedural misstep on a stab victim has terrible results. Outside of the ER, Carter (Noah Wyle) dashes off to Paris after learning that Kem's (Thandie Newton) mother has been hospitalized. Once there, Carter discovers some things about himself and makes some life-altering decisions. Also starring Sherry Stringfield, Goran Visnjic, Linda Cardellini.
