Chuck and Susan talk about the
realtor and buying a house. Chuck expressed that he doesn’t think it’s fair
that Susan had to work the night before and then has to go back in the
following morning. He tells her that she’s tired and Susan gets upset and asks
if she looks tired. Chuck tells her that she doesn’t as she puts on her jacket
and sees a vomit smudge on the lapel. She goes to change as Chuck tells her
that she worries too much. Susan explains that the board does not schedule
tenure meetings around the candidates. Chuck tells her she is over thinking
her wardrobe. Susan explains that with the tenure it will bring them financial
security, and they can buy a house and Cosmo can go to college. Chuck negates
saying ‘What if Cosmo doesn’t want to go to college’. Susan looks at him
oddly, while Chuck tells her that he can go back to work. Susan agrees and he
adds that she can work at a private hospital and she tells him that she
belongs at County. She puts on another jacket with a smudge on the back and
kisses Cosmo and Chuck goodbye. Chuck tries to tell her about the smudge and
she sighs while Cosmo laughs. Neela helps pull an 85-year-old man named Mr.
Rubadoux. Doris gives her the bullet as Ruby questions her on her medical
capabilities. A young man yells at John who is doing rounds residents and
interns. Ruby voices that he wants to leave but Neela tells him that they need
to do an exam first. He tells her that if he had any say he wouldn’t have come
to County in the first place. Ruby voices that he doesn’t want the doctors and
then sees John and states ‘especially him. I don’t want him anywhere near me.
He killed my wife’. John looks awkward upon seeing who it is as the doctors he
was with move along.

Sam rushes along and Pratt asks why his lymphoma patient
hadn’t been up to CT. Sam tells him she must have spaced out and would take
him right away. Jerry tells her not to bother that there was a long wait time
and as she turns to grab her charts, she spills a cup of coffee all over the
floor. Pratt starts handing out cases, giving Ray a patient that he would like
to trade with Abby. She tells him no, while Pratt hands Neela, Mr. Rubadoux’s
chart. Neela looks at Abby and tells her she’s done her shift and gives her
Ruby’s chart. Susan is in with the hospital board. They go over her
credentials but stumble on how she is not bringing in grant money. Abby
examines Ruby who is quite vocal about not wanting any of the doctors. He
tells her to contact his own doctor. She tells him that she can’t hear
anything and he states that she has been listening for five minutes. Abby
tells him he’s been talking for five minutes, which silences him because of
the tone of voice, she uses. Abby asks him if he was ever told he had a heart
murmur. Ruby looks at her confused as she explains to him what it is. He
questions if she thinks that is what he has and Abby replies that she is
pretty sure. Ruby sarcastically emphasizes ‘Oh you’re pretty sure’ while Abby
states that she wants to run some tests. Ruby scoffs that he doesn’t want some
nurse calling the shots when it comes to his health. Abby tells him that she
is not a nurse and listens to him continue to be ornery. Pratt and Sam bring
in a battered woman who is asking if ‘he is dead; Sam tells her that he is
right behind them and assures her that he will be in good hands. As the gurney
behind them passes she tries to jump off the gurney to attack a male that is
being wheeled into the next trauma room by Chuny and Luka. Sam questions the
female if her husband was the one that stabbed her and she replies that he
cornered her in the kitchen and held the knife to her throat. Sam pays notice
to the fact that she smells of alcohol. She continues to say that she didn’t
know what to do so she grabbed her husband’s gun from the kitchen drawer.
Pratt tells her that she has a collapsed lung, while Chuny comes in from the
other room and tells Sam that she missed restocking supplies. Sam tells her
that it was Malik’s turn. Chuny replies that according to the nurse’s schedule
it was her turn and they were all out of 8-0 ET tubes. Ray asks Chuny if she
took a vacation and she tells him that it’s not her fault that Sam forgot to
restock the shelves. Luka looks at her from across the two trauma rooms and
moves to get something from the cupboards. Pratt gets on Sam’s case about a
medication for a procedure. Luka tells her he was right there but she gets
upset saying that she will get it. Pratt asks the female to answer some
questions, while Luka gloves up to help. Abby presents Ruby’s case to John but
he tells her to get Luka to sign as the attending. Abby tells him he is caught
up in a trauma and tells him that the only way she could get Ruby to calm down
was to tell him she would get an experience doctor to see him. He reviews the
results, and walks in to see Ruby. John introduces himself and Ruby tells him
that he remembers him. Ruby asks him if he remembers him, and John plays
stupid. Ruby carries on to explain what happened 10 years ago. Haleh looks on
while John sits awkwardly listening to the old man express his love for his
deceased wife, whom John treated all those years back. John tries to take a
listen to Ruby’s heart but the old man pushes him away repeating that he wants
his own doctor. Abby tells him that John has to examine him and Ruby
surrenders. Abby and Carter walk out. Haleh follows and grins, saying that he
is as much of a handful now then he was years ago. John asks her if she
remembers him and she nods telling him not to let Ruby get to him. Susan
passes through admit. Ray sees her and asks her what she is doing there and
she tells him not to ask. Jerry tells her he tried to page her while Luka
states that they got slammed with a double trauma and could use the help.
Jerry tells her the residents meeting had been changed to later in the
afternoon. Susan scoffs that she just worked all night and can’t stay until
then and walks off displeased. Carter goes to records to pull Ruby’s wife’s
charts. He takes a seat and waits. Kaysen does a sonogram on Ruby’s heart
while the old man explains that his doctor thinks very highly of the
cardiologist. Abby speaks up and gets silenced by Ruby. Kaysen tells Ruby in
medical terms what is wrong, while Abby translates into easier English. Kaysen
continues to go through the treatment options and leaves to tend to other
matters while the tests are being done. Abby pulls up a stool while Ruby chats
about having a little more faith in Kaysen. Abby asks Ruby if he understood
everything Kaysen said. Ruby hesitantly tells her he did. She explains that
with the heart surgery there is risks, and that because of his age there might
be complications. Ruby asks her if she can fix his heart, to which Abby tells
him no. Ruby states that he will go with the guy that can and abruptly tells
her ‘bye’.

Abby, Carter and Kaysen head to Ruby’s exam room to get him
ready to go to surgery. Abby opposes that open heart surgery might not be a good
idea for Ruby. Kaysen replies that maybe that should be the surgeon’s call, but
Abby corrects him saying it’s the patient’s call. Pratt meets up with Olivia
outside the trauma room. They discuss the domestic violence case and Pratt leans
into asking her out. She tells him to pick her up at the office later. Ray
examines Birdie for a cough. They go on about how beautiful Abby is, while Ray
tells her that she will need to have a chest x-ray. Anspaugh draws a diagram of
what will be happening in Ruby’s surgery. Abby barges in and asks if it’s
definite that Ruby is having the operation. Ruby hesitantly agrees while Abby
tries to dissuade them from proceeding. Anspaugh excuses himself and urges Abby
to join him outside where he verbally lashes Abby for speaking about the surgery
in front of the patient. While Anspaugh returns to Ruby, Jake slips his hands
over Abby’s eyes and tells her that if he knows her as well as he does, that he
guess that Abby really hates it when people do that to her. Abby affirms it and
she turns to look at him once he removes his hands. She turns away and begins
walking away. He tells her that he is back and she apologizes for not noticing.
She asks how San Francisco was and he tells her that it was good. Medics bring
in a teenager who was luging down a main road and bailed out, hitting his head
on a wall. Luka and Sam take it and they take him to a trauma room. Pratt asks
him how fast he was going and the kid tells them 40-50 mph. Pratt replies with
an ‘Alright’. Sam scoffs and Luka asks him where he found hills big enough to
luge down and Sam scoffs once again saying that the kid rammed his head into a
wall. The kid and his friend high five and the patient zones out saying that his
hands are numb. Luka orders a C-spine and they move him onto the other bed. Abby
catches up with Carter and wants to talk about Ruby. Ray interrupts to present
the Birdie’s case and reminds Abby that she treated her last week. Carter tells
him to treat her like an outpatient and put them into family care. Abby says her
sister and her are panhandlers and have no family. Ray states that panhandling
pays the bills and Abby looks at him funny. Carter suggests Ray call Social
Services and get them to come down to see what they could do about elderly
assistance. He begins walking away and Abby tails him still wanting to discuss
Ruby. Chuny interrupts while Abby is trying to get John in on helping her talk
to Ruby about the surgery. He states that she is his doctor and she replies that
he is the attending. Frank announces that ‘it’s that time of year’ and he has
Girl Scout cookies. Carter looks excited while Abby comes in before Carter signs
for some boxes and pushes Frank away. Abby asks why John keeps trying to blow
this off and he replies he isn’t. John tells her that every once in awhile she
will come across an old man that just feels more comfortable hearing things from
a grey haired doctor and that it is up to her to get through to him. She tells
him she has and he suggests she move on.
Frank wakes Susan up and tells her that the Chief Residency
meeting is starting and she thanks him. After seeing her not really moving, he
asks her if she’s really up because he has to leave. Susan gets up and walks
through and sees Pratt trying to saw through the luger’s helmet. She asks them
what they have and Luka hands her the chart. Abby comes in looking for Carter
and Susan tells him he’s probably at the meeting where Luka and her are suppose
to be. Abby asks for a central line on Ruby, and Luka sends Pratt to help her.
Morris walks in all dressed up and Carter pays him a sarcastic compliment. Susan
comes in, Morris asks her to dim the lights and he proceeds with his
presentation. Ray bolts into Birdie’s exam and tells them they can’t leave.
Birdie’s sister, Rebecca tells him they can and they are. He apologizes for the
backup and Social Services running behind and the Rebecca tells him they saw
them last time. Ray gives a rather fake explanation on why they have to stay but
the sisters call his bluff. Radiology comes to get the luger and Luka is left
with Sam in the trauma room. He tries to ask her what is going on and she speaks
tersely that there is nothing wrong and she is just cleaning and restocking. He
asks her if this is about the luge boy and she goes off saying that he was
encouraging it all. He tells her that he wasn’t and that she was overreacting.
She turns and throws some towels at him. Stifling her laugh she walks into his
arms and tells him that she is having a bad day and has a headache. He tells her
to take off and says that if anyone has a problem with it to have him or her
come to him. He kisses her on the forehead and then walks out. Pratt sets Abby
up for Ruby’s line and asks him how he is doing. Ruby states that he doesn’t
know and for Pratt to tell him. After Abby inserts the guide wire, Ruby’s heart
begins to race and Haleh states he is throwing PVC’s. Pratt tells Abby to pull
back and the chimes from the machines keep beeping. Haleh tells Abby to call a
V-tach and Pratt urges her. She tells him she is nowhere near the myocardium.
Haleh calls V-tach again and Abby pulls out. With no response from Ruby, Pratt
orders they get him into a trauma room immediately. Morris continues with his
presentation, while Carter and Susan try to stay awake. Carter’s pager goes off
and he excuses himself. Ray enters Birdie’s exam and sees an unfamiliar face.
The lady thanks Ray for taking care of them, and Birdie says that since the lady
is there that they can leave and go with her. The lady asks Ray if they can talk
and he takes her outside. She explains that she has been trying so hard to get
them to come live with her and her family, but that they are stubborn and want
their independence. Ray tells her that he will talk to the sisters and heads in
to do so. Pratt shocks Ruby while Carter races in and tells Abby to switch out.
He takes over compressions while Pratt brings him up to speed on what happened.
While Pratt seemingly agrees with Abby that Ruby shouldn’t be having the
surgery, John makes it known that he thinks he broke the old man’s rib. Pratt
shocks him again and a rhythm is detected.
Ruby wakes up, and while Carter watches through the glass of
the doors, Abby notices him. Susan and Kerry walk out of the elevator and talk
about the roles and expectations of Chief Resident and how Morris would be the
best pick. Susan states that the med students think he’s a joke and that they
should get someone from the outside. Luka approaches and asks how the meeting
went and Kerry explains that he is a possibility. Luka asks if she is joking and
Kerry explains that if he wasn’t there that he can’t complain. Luka tells her
that he was in a trauma and that they should be able to vote. Susan complains
that she’s been up for 36 hours and has to go home and Kerry leaves also. Luka
tracks down Carter and asks what happened. John says he left and doesn’t seem to
care much. Frank gives Luka a chart and he goes to take care of it. Abby
approaches and tells John that she told the chiefs about the broken rib and
tension pneumo and he questions if they are still going through with the
surgery. She states that they will be waiting a few days until the pneumo
repairs itself. Pratt enters the Cease-fire building and asks the main desk
clerk where he can find Olivia. He takes Pratt to her and it seems she is in
some community group meeting. She notices him and introduces the group to him.
She goes on to explain what they are talking about and then puts him on the spot
by asking him to say something on ‘jobs and healthcare’. Seeing that he was put
on the spot in a sweet malicious way, he laughs and tries to think of something
to say. Kids run around Luka while he tries to explain the treatments of 2
children to the mother. Sam sneaks her head in and he sees her and excuses
himself to talk to her. She tells him she is off and he asks her if she wants to
grab a bite to eat when he gets home. She explains she is tired and suggests
staying home. He agrees and kisses her goodbye. John grabs a coffee from the
food guy outside in the bay. Abby is there also and she warns him about how bad
the coffee is. Before walking back inside, Carter stops to explain the
connection he has with Ruby. He says in the end ‘that ultimately it was the
attendings call – it wasn’t even my responsibility’ and walks off to greet an
incoming. He calls out asking if she wants to help with the new patient and she
goes. Pratt talks to some people after the meeting. A man asks him if he is in
any relation to ‘Charlie Pratt’. Olivia knows of him also and Greg looks
intrigued but shocked. Olivia states that Greg is still speaking to her and
rambles on about misleading him about dinner. She suggests dessert instead, but
Greg doesn’t seem to be paying attention after being asked about Charlie. She
asks him again and he tells her that he has to go check on a patient in the ER.
He says he will call her and walks out. Sam stands in the aisle of a grocery
store looking at the pregnancy tests. She grabs one and takes it to the register
and pays. Ray heads out but before he can actually get out, Carter asks him
where he is going. Ray explains that he knows his shift doesn’t end for another
half hour but he filled in Abby on all his cases. Carter questions if Abby is
his attending and knowing he was right, Ray proceeds to fill Carter in, and adds
that Birdie and Rebecca left with their niece. John asks what Social Services
said, and Ray replies that they didn’t make it down in time. John starts
overreacting about the sisters and Ray looks on confused. It seems John is
taking the Ruby case too seriously and making sure that he doesn’t let the same
thing happen to his med students. He proceeds to walk away and in direct line of
the trauma rooms. He enters Ruby’s room and begins telling Ruby how sorry he is.
They talk about Ruby’ wife and the mistakes Carter made with her and Ruby. He
sits down and tells Ruby about the possible outcomes of the surgery Ruby is
scheduled for. Ruby listens. Carter finished by telling Ruby ’10 years ago I
lied to you about your wife… But I am not lying to you now’.

NBC Review
HEALING OLD WOUNDS IS NOT EASY AT THE ER - 85-year-old Jules
Rubadoux (guest star Red Buttons) is wheeled in and erroneously accuses Dr.
Carter (Noah Wyle) of killing his wife ten years ago. Although Abby (Maura
Tierney) believes Rubadoux is too old to undertake surgery to correct a heart
murmur, the attending doctors opt to go ahead with the operation. Meanwhile, Dr.
Pratt (Mekhi Phifer) treats a domestic violence victim and makes a date with the
social worker assigned to her case. As Pratt arrives for the date he realizes he
has gotten himself into more than he bargained for. Also starring Shane West,
Sherry Stringfield, Linda Cardellini.
