Pratt lies in bed as he
listens to the shower running. A female is making noises from inside the
bathroom and emerges saying the hot water ran out. Pratt her invites her into
his blankets telling her he will warm her up. After they kiss, he suggests
they call in sick and she tells him she can’t. He asks her about later and she
explains that it is her friend’s birthday and her and some other friends are
taking her out for dinner. She suggests the next night and he tells her that
after that day, he is on nights for a week. She rolls away from him and tells
him that she didn’t think the relationship would work between them because she
is looking for someone that is further along in his life. Carter distributes
charts to the residents, while Pratt and Sam walk in. Ray tells her that he
needs an IV started on his patient and Pratt interjects asking Ray how he’s
suppose to learn how to start a line if he keeps asking other people to do it.
Ray states that he thought IV’s were a nurse’s thing and Pratt dismisses that
by telling him everybody has to do his or her own procedures. Carter overhears
and repeats it for everyone to hear. Jake takes the chart from Ray saying he
needs the practice since he will be starting his surgical rotation in a few
days as Abby overhears. Ray complains about the rule as Susan complains that
she can’t make up the schedule because there is a shortage of attending's.
Pratt asks Sam to order a head CT for his vertigo patient and she tells him he
needs an attending to sign off on the order. Susan hears this and tells Pratt
that they are cutting down on unnecessary head CT so that they can order a new
scanner. Pratt states that his patient is necessary and she asks him how,
considering he was only in the exam for 2 seconds. Pratt tells her that he may
have a cerebellum mass and she interrupts saying that it may only be an inner
ear infection. She orders him to do a neuro exam and present it to an
attending and they would decide if a CT was necessary. Pratt tries to get
Carter to side with him but he tells Pratt that ‘it’s her ER’ and Susan adds
that it sucks to be him.

Pratt tends to a patient who is drug seeking. Morris calls
on him to help him with a patient named Rocky who says she knows Pratt. He
describes her as lonely and will do whatever it takes to stay in the hospital
to talk to someone. He tells Morris to page Wendall. Susan approaches Pratt
telling him she has a report to do for her chart review. He wishes her good
luck and she adds in that it is for both of them to do. Pratt looks at her in
disbelief and she tells him she will add his name on the report. He tells her
he’s not interested and she explains he should because in a few years he will
be looking for a job, staring at a blank spot where he should be listing his
articles. He smiles, and tells her that he is writing an article and it’s on
‘The Abusive Powers of Attending’s on Residents’. She gives him the papers and
walks away. Abby is going over the chest films with Jake, who is obviously not
focusing. He asks her if the problem lies with the fact that he is her
student. Abby pretends like she never heard him and heads towards admit. She
tells him to stop talking about it or she will give him a failing mark on his
evaluation. They both enter the room where an older lady is complaining of
nausea. She tells them that her heart doesn’t feel right and Abby orders an
EKG. Pratt examines a boy named Thomas. Thomas’s stepmother says he didn’t
want to go to school because he didn’t feel well. As Thomas lifts his shirt so
that Pratt can listen to his chest, he notices a rather large laceration
running across the boy’s stomach. He asks the mother what happened as she
doesn’t know and asks Thomas the same question. Thomas shrugs and looks at his
brother, then tells them he can’t remember. Susan asks Jane to take inventory
of all books and computers for a pharmaceutical grant she’s applying for and
Jane agrees to do it. Wendall talks with Rocky who is quite depressed. Ray
tells her that he has her discharge papers and she seems upset that she has to
leave. As they walk away together, Wendall asks why she was paged for Rocky
and Ray tells her that she needed to talk and that he had work to do. She
tells him she is not a babysitter and didn’t appreciate being called for that
reason. Pratt walks by her and asks her to talk to Thomas and then runs with
Luka on his way to a trauma room with a GSV. Wendall tells him they should go
in together. Pratt tells her he will be in shortly. As they move the patient,
who’s name is Martin over to the bed, they come to find out that he is quite
the talker. A police officer handcuffs the patient to the bed and Pratt asks
why. The cop tells him that Martin is being charged with assault. Wendall
talks with Thomas about how he got the cut on his stomach and he tells her
that he got it from climbing a fence at school. He tells her it was a barbwire
fence and that he ripped his shirt. He also adds that the reason he didn’t
tell anyone was because the shirt belonged to his brother. The stepmother
voices her disappointment, while Thomas’s brother says he doesn’t care.
Wendall asks if she can talk with Thomas alone and the mother asks if that’s
necessary, she states she is already late for work. Sam tells her that he
can’t be discharged until they get his test results back. Wendall gets paged
and excuses herself. Abby and Jake go over the EKG on the old lady. As Abby
walks out of her exam room, Susan stops her and tells her that she will have
to start noting start and stop times on all documentations. She also asks her
to get the rest of the house staff to fill out the surveys and get them back
to her before the end of the day. As she walks into admit, John stops her and
asks her what the deal is on the time surveys. Susan tells him that she is
applying for a grant with a pharmaceutical company. He states that he thinks
the residents should be treating patients and not doing free research for the
companies she’s applying for. Susan tells him that if he didn’t spill his guts
on TV about the zirictal she wouldn’t have to do this. John defends himself
saying that he was right and Susan tells him that he cost County $50,000 in
books, computers and educational literature. She also adds that County State
Hospital is ‘bleeding out’ and unrestricted grants from pharmaceutical
companies pays the bills. Carter denies there is no such thing as unrestricted
grants and that it leads to a conflict of interest. Martin looks around as
Luka, Pratt and Sam treat him. Dubenko arrives and asks why they are looking
at his stomach when he was clearly shot in the chest. Luka tells him that the
bullet may have ricocheted and landed somewhere else. Dubenko makes his
orders, while Martin asks if he needs an operation. Dubenko tells him no but
Luka tells him he might. Luka tells Pratt to order a CT, and Dubenko voices
that Luka should trust his clinical judgment. Luka says he does and wants the
CT done and that Martin is his patient. Dubenko calls out some orders and
writes them in the chart and tells him he just admitted Martin to the surgical
floor and his care and that all they are doing is holding him until a bed is
ready.

Sam and Luka get coffee from the grocery store across from
the hospital. Luka ponders Martin’s condition out loud. He asks Sam if Dubenko
gets on her nerves and she comments that she found him kind of cute. She adds
that he acts the way he does because it is a defense mechanism to look better to
the staff. Carter takes on a new patient that has been impaled by an arrow.
Pratt discharges Thomas while Wendall overhears and asks them to wait a minute.
She tells Pratt that she wasn’t finished with her assessment on Thomas. He tells
her that she said that she found that Thomas’s injury was consistent with his
story. Wendall interrupts saying that if he pages her for a consult to at least
finish. Morris walks up asking Wendall to take a sexual history on a patient of
his. She gets angry saying that she’s sick of doctors using social work as a
crutch for when they don’t want to talk to their own patients. The ‘arrow’
patient asks when they will take it out and Carter tells him that they would
have to do the removing in the OR. Nor Abby or Ray can find a vein to put in an
IV so Sam takes a foot and tries inserting one there. Jane tells Abby that she
thinks its cool that she lets her med students do procedures. She also adds that
is the main reason why she switched shifts with Jake. Pratt and Luka talk about
the procedure to find out if Martin’s bullet did ricochet. Carter goes into the
lounge to get some coffee and runs into Susan. He apologizes to her for not
being the best team player while Susan also apologizes for getting upset and
taking it out on him. Wendall approaches Carter saying that she won’t be taking
any more consults for the ER that day. Carter asks why and Wendall expresses to
him that if the doctors need help with their patients they can call a candy
striper. Pratt and Luka do the procedure on Martin but Chuny calls Luka away.
Martin begins to explain why he got shot but Dubenko comes in asking Pratt what
he’s doing. He understands without Pratt saying a word and begins to prepare for
an ex lap. Pratt asks what he is doing and Dubenko tells him that if they are so
sure Martin needs an ex lap then they would perform it there in the trauma room
and tells Pratt to gown up to assist him.
Pratt finishes up the ex lap with Dubenko and Martin asks
what happens next. Pratt tells him that gravity will pull the fluid out and if
blood comes out then he will need surgery. Dubenko argues that all doctors ever
do is jump to conclusions while Pratt defends Luka and his thinking. Luka
arrives and tells Dubenko if he has anything that he wants to say then to say it
to him and not his residents. He tells Dubenko that in the years he’s been
treating the patients that surgeons blow off, he’s developed a sense of
intuition. Dubenko replies that he likes to practice evidence-based medicine,
and begins to review Martin’s case. He pulls up the catheter bag, which has a
pinkish tinge. Martin asks if that’s bad and Dubenko tells him no, that it was
most likely a tablespoon of blood that was caused from Pratt making the
incision. Luka adds that it could also be from a tear that was intermittently
oozing. Dubenko tells them that he is getting tired of listening to the
arguments and leaves saying he will find Martin a bed himself. Abby and Jake
wheel Gerty, the old lady with chest pains upstairs to cardiology. Abby explains
another procedure they will do. Jake asks Gerty if there is anyone she would
like him to call, but she says there is no one close by. Jake tells her that he
is almost off and would come and keep her company while they do the procedure.
As they walk out, Abby tells Jake that she thought that was nice of him to
volunteer to sit with her, and that it was probably a good idea that Jake
switched shifts because it gave him a little variety. Pratt tells Luka that the
next time him and Dubenko have a turf war over a patient to leave him out of it.
Luka explains it’s not about power but about the patient. Pratt asks him what
makes him think that he is right and Dubenko is wrong, and Luka answers saying
you just learn things over the years. Jake walks Abby to the elevator and before
the doors can close, Jake stops them and tells her the reason he switched shifts
with Jane. He explains that he is no longer her student and that she should fill
out his evaluation and seal it, because they were going to go out the next
night. She tells him no, he says yes and finally she get the last no in before
the doors fully close. Jane asks Pratt if he ever considered surgery, and he
tells her no, that a month on rotation in the surgical ICU was enough. Sam asks
Pratt if yawning is a sign of anything and he replies that it’s a sign of being
tired. She tells him that Martin seems wired and that he’s doing it a lot. Pratt
tells her to call Dubenko and goes back to Martin. Ray and Morris meet EMT’s
with Thomas who was found on the school playground unconscious. Ray calls in
Carter while they wheel him to one of the trauma rooms. Morris tells someone to
page Wendall and Ray asks why. Morris tells him that her and Pratt saw Thomas
earlier. Martins head gets lowered while they prep him, while Luka arrives to
ask Pratt what is going on and Pratt replies for him ‘to watch and see’. Martin
asks to make a call and Jane asks him for the number. Dubenko wanders in to ask
what is going on now, while Pratt tells him that Sam noticed Martin yawning
frequently. Dubenko tells Pratt he has 30 seconds to explain. Pratt continues to
say that it could be a sign of a diaphragmatic injury. Martin talks with ‘Sally’
and tells her that her husband is cheating on her and that he loved her and
hangs up. He tells them that she is his boss’s wife. Dubenko says that he will
take Martin upstairs now but Sam interrupts saying that the fluid output shows
that Pratt was right. Ray calls Sam over to help with Thomas, but she doesn’t
know it’s him. When she emerges she is shocked to see it’s him. Wendall also
arrives and is equally shocked. Pratt gloats to Dubenko as they wheel Martin to
the elevators to take him to the OR. Dubenko tells him he should write up the
case. As he watches them leave, Pratt sees Chuny and asks her what is going on.
She frowns saying ‘abused kid.’ Pratt goes in to check it out and sees Thomas
laying on the gurney. He tells Ray, who is doing compressions to switch off.
Wendall says she shouldn’t have let them leave until she had the chance to talk
to Thomas alone. Carter asks for the ETA on Thomas’s family, thinking they are
already too late to do anything more, but Pratt urges them to continue because
he is just a kid.
Pratt gets told that they had been resuscitating Thomas for
43 minutes. The family arrives and Sam goes out to tell his father and
stepmother that his injuries are severe. She asks them if they would like to see
him. Sam spots Casey, Thomas’s brother and Wendall asks him what happened. He
tells her that he saw ‘them’ drag his brother behind the swings. Wendall asks
whom and he tells her a group of kids that seemed to have a thing with the way
his brother dressed. He says that’s the reason he lent Thomas his shirt that
morning. Wendall urges him to tell her who they were and promises he would be
safe. He shakes his head and tells her that he sees them every day in school. As
he looks in on his brother, he tells them that they are 6th graders. Carter
tells Pratt to hold compressions, but he doesn’t want to give up. Carter stops
him and then tells the parents he is sorry, but Thomas’s injuries were too
severe. Abby sits in the lounge looking at Jake’s evaluation. Susan steps in and
they talk about Carter’s press interview. Pratt looks at Thomas’s father who is
sitting with his deceased son. Luka walks up to him and tells him that they
found Martin had a lacerated diaphragm, and that Pratt saved his life. Neela
arrives and Abby signs off on her patients. Sam invites anyone to join her for a
drink. Susan accepts while Jane hands her the inventory on the computers and
books. Susan offers to buy her a drink but Jane declines asking for a
recommendation instead. Susan agrees while Sam invites Ray to come along. He
stops in front of Neela and she tells him that the plumber was at the house and
he owes her $80. He tells her he loves her and gives her his charts. Wendall and
John walk down the street, she tells him not to say anything and that she should
have finished her exam on Thomas. He tells her that she did what she could and
she tells him to stop trying to cheer her up. She breaks down and hugs Carter
for comfort. Pratt arrives at the restaurant where the woman he was with that
morning is. He goes in and tells her that he thought a lot about what she said
to him that morning. He tells her that what he does is as real and serious as it
gets and that he is very much grown up. He apologizes for offending him as he
goes on to wish her well. Abby arrives home and sees Jake standing on her step.
He tells her she forgot to sign off on his ABG and his placement. She states
that he stalked her out to her apartment and he adds that student/teacher
relations need to be wrapped up by the next day. She signs his book and there is
a pause before she leans in to kiss him.

CUTTING CORNERS COSTS LIVES IN THE ER - While examining a
young boy, Pratt (Mekhi Phifer) finds a suspicious gash across his chest and
calls Wendall (Madchen Amick) for a consult. Kovac (Goran Visjnic) and Pratt
examine a man who got shot when he confronted his boss with a gun. Jake (Eion
Bailey) tries to convince Abby (Maura Tierney) that being a student shouldn't
keep him from dating her. Also starring Sherry Stringfield, Laura Innes, Noah
Wyle, Shane West, Parminder Nagra, Linda Cardellini.
