Susan receives a fax while talking on the phone with Kerry. Luka walks in asking if the fax was the patient satisfaction scores. She nods yes, while still listening to Kerry’s concerns. Luka asks how they did and Susan rolls her eyes and shakes her head. As she hangs up, Luka guesses the person Susan was talking with was Kerry and she relates the scores and tells Luka the patients were upset that the residents of County didn’t listen or seem to care about the patients. Susan rattled off individual scoring of Pratt, Ray and Neela. Luka asks about Abby’s scores. Susan tells him and relates also that Abby has other issues she’s more concerned about. Luka tells her that they need to be taught. Susan questions how they can teach empathy. She sees Ray walking out of an exam wearing street clothes and his tattoos showing. Using the same breath she adds ‘or common sense’. Susan breaks into Chief mode and tells Ray that when patients come in, they want to see a doctor and not a rock star. She directs him to change into some scrubs. Susan tells everyone around her that there is a dress code. She sees Pratt wandering in 4 minutes late. She begins to lecture him only to find in the end he hadn’t heard a word she said. She shifts over to Abby to discuss a complaint made by a renal attending about dialysis that Abby ordered a couple weeks prior. Susan tells her that in order to appease them she agreed to review 3 months worth of Abby’s patients. Susan breaks into some of the discrepancies she had found and Abby takes it personally. Abby explains one chart and then asks Susan who she is? Jerry alerts them to an incoming MVA trauma while Abby calls Susan on her new ‘leadership attitude’ and why she would falsify legal documents. Abby grabs her jacket, while Susan asks her where she is going. Abby replies that she is on break, and Susan informs her that residents don’t get breaks. Abby changes her answer and tells Susan that she is going outside to wait for the MVA. Abby steps out into the ambulance bay and lights up a cigarette. As she gets more annoyed a black SUV races into the bay and stops. A kid jumps out calling for help and Abby races over asking what happened. As the kid tells her someone shot his brother, Abby looks into the vehicle and gets pushed in and the car speeds away.

 

Abby fights with the kid in the backseat while they drive down the streets of Chicago. The kid calls out ‘Loose’ (the driver) and he stops the car. He turns and points a gun in Abby’s face. She calms down and sits still, while the kid tells her that his brother CJ is in the back. Loose tells her to get in the back of the car. She obeys and climbs into the back to discover CJ trembling and bleeding profusely. He tells her that they shot him from behind and it came out the front. Abby lifts his shirt and sees the gapping whole in his stomach. The kid yells at her to fix him and she tells him she can’t do anything and CJ needs a hospital. The kid points the gun to the side of Abby’s head and repeats for Abby to fix him or he would shoot her in the head. Sam puts oxygen on a lady who’s complaining of shortness of breath and tells her to remain calm. The lady tells her that her regular doctor is out of town and she is pretty particular on who she sees. Sam tells her that someone will be along shortly to examine her. The lady redirects her attention to a crying Hispanic lady with a splinter in her toe. Ray, accompanied by 2 med students tries to calm her down. As they finish up Ray leads them over to the lady with shortness of breath. He grabs ‘Ms. Mitchell’s’ chart and discusses her symptoms. He tells her they will run some tests and come back to fully examine her. Sam returns and Ms. Mitchell asks about the possibility of getting a different doctor. Abby tells the kid that his brother has lost a lot of blood and voices there was not a lot she could do. She looks at Loose through the rear view mirror and then back down at CJ. Abby tells them that she will try to stop the bleeding and asks if they have any towels or napkins. The kid asks CJ, to which he tells her to use anything until they can get some supplies. Abby tells CJ they need more then supplies that he needs surgery. She takes off her lab coat and applies pressure to CJ’s wound. CJ tells her to make a list of the things she needs. Loose pulls up to a drugstore and tells the kid that if Abby tries anything to shoot her. Abby panics and tells CJ that she can’t just patch him up; that he has internal injuries that he could bleed to death from. CJ tells her that if she had to use a needle and thread to close him up then that would have to happen. Loose tells her to start writing out a list. Neela discusses a patient with 2 med students while another patient in another bed asks her about his cancer. Neela confuses him with medical jargon and catches herself. She tells him in simpler words, while he breaks into tears. Neela shows no emotion and leads the med students out of the room. Luka eavesdrops as Neela reverts to med-speak again. Luka looks through the exam room window sympathetically at the crying patient. He interrupts Neela who is asking the students about the differential diagnosis of crying. Luka interrupts and tells Neela that the man just learned his cancer was spreading and that should be reason enough for him to cry. Neela explains there could be other medical reasons associated with crying. A nurse interrupts to introduce a man that had lost his wife that week and to ask if either Luka or Neela worked on Monday. Neela informed her that she worked that day but didn’t remember the name he provided. Luka advised that he check the nurse’s station, which someone there should remember and be of better help. The nurse takes him over to John and Wendall and explains the man’s loss. John apologizes and the man thanks him. He tells John that he was out of town and his wife was alone. He asks if his wife said anything or showed fear. Wendall asks him what his wife’s name was and tells him she would try to find out for him. She tells him it might take her awhile and where she can contact him. He asks her if he can just stay there at the hospital and Wendall and John both nod. He pauses to ask if there was anyway they could find out which room she was in but John tells him they don’t keep that information. As John and Wendall walk away, Wendall begins to ask him something personal. Susan interrupts asking if they had seen Abby. Carter asks what happened and Susan tells him Abby went to wait for an MVA and has been missing ever since. Susan asks Malik at admit to text message Abby and tells her to get her butt back to work. Abby’s pager goes off while she waits for the supplies. The kid asks if she has a cell phone and she tells him she does but doesn’t have it with her. The kid tells her his brother has one and when it rings it plays music. He tells her that his brother is the captain of the ship. Abby asks him his name and he tells her that most people call him Lil C. He asks her why she asked and she tells him that Loose thinks she can help CJ, but she says she can’t and CJ will die if he doesn’t get an operation. Loose returns with the supplies and tells her to get busy. A gang victim is brought into the ER and Susan takes it. She asks the patient her name and the girl cries out that she was only waiting for a bus. Abby’s pager goes off again as they speed through an alley. Abby tells Lil C that she can’t see anything so he turns on the dome light and asks what she will do first. Abby tells him that she has to move his intestines back in and Lil C asks if it will hurt CJ. Abby tells him it will. As Abby is being bounced around in the SUV she tells Loose she can’t do anything while they are moving. Loose comes to a stop and tells them he thinks he hit something. He reverses and there is a cat lying in the middle of the road. Lil C jumps out and inspects the cat saying he still thought it was alive. Loose gets out, takes the gun from Lil C and shoots the cat. As Loose and Lil C climb back into the truck, Abby’s pager goes off again and CJ takes it from her, passing it to Lil C, who throws it out the window.

 

Loose comes to a stop in a dark corner of the back alley and opens up the back of the truck. Abby tries to give CJ some cough syrup with codeine to dull the pain before she can stitch him up. As she starts pouring it in his mouth, he coughs it back up. Loose takes the bottle and props CJ up more and tells him to drink it all up. They lay him back down, covering him with available jackets and a blanket. Loose sees CJ’s gun and looks at Abby. CJ pulls him down telling him not to be stupid and to pay attention. CJ urges Abby to start but she tells him they need some time for the codeine to take effect. Neela, Susan, Luka, Pratt and Ray are working on the girl who was waiting for her bus. Carter emerges to tell them another gang member is coming in. He asks if they can spare a resident and Ray volunteers. Luka tells Carter to take two and sends Pratt along. Susan looks at him like he’s out of his mind. Sam walks through curtains and sees the husband of the wife that died sitting by an empty bed. She asks him what he’s doing. He replies that he thought that’s where his wife had been and that’s where he felt her presence. He questions Sam, asking her if she remembers his wife, and she tells him no. Abby threads a needle and tells them all how stupid it was. CJ tells her he feels better and she tells him it was pointless of her sewing him up because he needed surgery. CJ tells her to do it so she begins by saying to brace himself. Ray tries to make the gang member feel comfortable by talking like he’s from the hood. The gang member tells him he sounds stupid but when he hears Pratt talking he takes an instant liking to him. A nurse comes in to tell Ray that the splinter lady is looking for him and he makes the moves to leave but Carter tells him they haven’t even rolled the patient yet. Ray tells him the gang member feels more comfortable with Pratt and he excuses himself. Loose asks Abby how CJ is and she tells him he will die if they don’t get him to a hospital. Abby asks what the plan is next and Lil C tells her once the cops are gone they would take CJ home. Loose looks at Abby through the rear view mirror and rolls his eyes, saying they would go and get some food. While on the phone, Susan tells Neela that she needs to talk to her about her scores. When she gets off, she hands Neela a chart and tells her to be nice and comforting and stay in the room longer then 2 minutes. Neela tells her she has other things to do and Susan replies by saying she wasn’t asking. Sam tells Susan that Ms. Mitchell is still waiting to be seen and that she was Ray’s patient who asked to be seen by someone else. Susan asks Ms. Mitchell about her ailments while Sa, looks at her chest film. Ms. Mitchell wants to tell Susan something but feels uncomfortable with Sam there. Susan asks Sam to go check on some other results, so Ms. Mitchell can say what she wants. Ray comes in and sees Susan with his patient and approaches to tell Ms. Mitchell that she is pregnant. Susan pulls him away to tell him that Ms. Mitchell is actually a pre-op transsexual. Ray is confused about the positive pregnancy test, but Susan is quick to tell him that with Ms. Mitchell’s test results that it can only mean one thing. At the drive through, Abby looks around at what’s going on outside and in the restaurant. Her eyes settle on the latch for the back door. She eases over CJ to try an escape but CJ catches her. Wendall asks John to go snowboarding with her but he is quick to tell her he has other plans. Neela interrupts and asks john if she should be worried about Abby and he assures her Abby can take care of herself and she is most likely just taking a walk. Abby tells Loose that she needs to use a washroom. Loose drives into a dense wooded area and stops. He opens the back door and Abby tells him it’s ok she doesn’t have to go. He tells her to get out and if she needs to go, she won’t be doing it alone.

 

Sitting on the tailgate of the truck with Lil C, Abby checks on CJ’s vitals. As she is pulling up his sleeve she sees a tattoo. Lil C shows her the same tattoo only on his arm and she asks what it means. He tells her he’s not sure but he would find out someday. Loose is on the phone with someone and Lil C reads a comic book. When he gets off the phone he tells Lil C to hand it over for the fire. Loose lights up a cigarette and Abby asks for one. He tells her smoking can give you cancer. Luka and John discuss Abby. Susan steps up behind them and asks them if they finished some reports. They tell her no and she pulls rank again saying that if she sets a deadline it needs to be honoured. Haleh comes to tell John the gang kid is crashing. Lil C tells Abby that he thinks the fire worked and CJ had stopped shivering. Abby jumps into the back of the truck and tells them he has no pulse and isn’t breathing. She starts compressions. John tries to hold down the gang kid who is seizing, they stabilize him and send him to CT. Abby continues with the compressions and tells Loose they need to get CJ to the hospital. Pratt and Sam wheel the gang kid back from CT and Pratt tells Luka that the CT showed a bleed in his head and OR says it’s inoperable. Luka tells Pratt that they need to decompress the brain before he herniates and dies. Luka tells Sam to page OR and she tells them they are busy. Luka decides to do a procedure right there in trauma and tells Sam to get Neela. Pratt tells him he’s only done the procedure once and Luka tells him Neela hasn’t done it yet. Abby tells Loose she can’t do the compressions herself and he pushes Lil C forward. Abby shows him how to do it and explains that with each 5 compressions she will breath into CJ. Sam returns to the trauma room with Neela and Luka brings her up to speed on the procedure they will be doing. Luka walks her through and Wendall interrupts saying the man that lost his wife was Neela’s patient. Abby and Lil C continue the compressions on CJ. A tired Lil C pulls back and says he can’t do it anymore. Abby continues, doing the 5th compression, Loose breathes for CJ. Ray approaches Ms. Mitchell and tells her that she has testicular cancer. He explains that in pregnant women a hormone called Beta HCG is present in the urine but in males it supports testicular cancer. Ray tells her it’s treatable but she laughs saying how ironic it is that she was getting rid of the ‘parts’ anyways. Ray asks why she wanted another doctor, and she tells him that she doesn’t trust people. Abby and Loose continue to try and revive CJ. Lil C asks if his brother is dying and Abby says yes. He yells at her saying that his brother isn’t going to die. Loose gives up, but for fear of her life Abby continues even though she knows it’s pointless. Loose covers CJ’s face with a blanket and he retreats to kneel by the fire to think about what to do next. Abby and Loose exchange glances. He stands up holding the gun down by him and Abby looks at him scared.

 

Loose tells Abby to get back into the SUV and she does panicked. She tells him he doesn’t have to do this. Lil C closes the door and Loose tells her to lie down. She complies again. Luka and Neela finish up with the procedure on the gang member. They walk out of the trauma room and Luka tells her she is a gifted confident doctor when it comes to procedures, but she needs to learn to take that confidence and move it over to the other half. Neela doesn’t understand and Wendall walks up behind them handing Neela the chart on the wife that died. They stop in front of the exam room where the husband is and Neela confesses she doesn’t know what to say to him. Luka tells her she does and Neela tells him that she has no memory of the patient and wants to know if she’s to lie to him. Luka tells her to trust herself enough to know the words to comfort him. She opens the door and lets herself in. She apologizes for not remembering his wife’s name. He tells her its ok and asks her if she remembers his wife now. Neela tells him that his wife talked about him and how much she loved him and that she thought she wasn’t afraid. John races up to Wendall announcing his plans have changed and he would still like to go if the offer was still available. She turns to him asking if he is ok with moving to the next step of their relationship and he tells her yes and asks if she is. She tells him yes also. Susan and Pratt walk and discuss Luka’s procedure. Susan sees John and asks him why Luka let Neela do it. John doesn’t see the big deal, but Susan is plainly annoyed that she wasn’t talked to about it. She tells John that she was going to talk to Luka about it, but John tells her she is overreacting. Abby watches the reflection of the passing buildings in the window as she lies down. She tells them that she promises not to tell anyone but no one answers her. She tells them she just wants to go home. Susan is sitting in the lounge with a mound of charts surrounding her. Kerry walks in and asks her how’s it going. Susan tells her ‘ok’ but then retracts by saying ‘it blows’. Susan tells Kerry that every time she turns around she’s either hurting someone’s feelings or being a crab. Kerry tells her it sucks to be in charge and Susan asks what to do. Kerry tells her to get used to it. Susan is shocked that is all Kerry had for advise. Kerry tells her that as a woman in charge she has to be prepared to piss people off and lose a lot of friends. Kerry walks out and Susan gets on the phone dialling Abby’s place. She tells Abby’s answering machine that she is worried about her and apologizes for earlier. Abby continues to watch the reflections through her window still lying in the truck. They come to a stop and Loose opens her door telling her to get out and there was business to get to. She gets out and turns to see County across the street. Lil C thanks her and they speed away. Abby watches them leave, and then turns to walk back into County cold, shaken and relieved.

ABBY IS KIDNAPPED BY GANG MEMBERS IN THE HOPES THAT SHE CAN SAVE ONE OF THEIR WOUNDED - As Abby (Maura Tierney) smokes in the ambulance bay, an SUV pulls in carrying a gunshot victim. When Abby runs to the car to help the patient, she's pulled inside at gunpoint and driven away. Ray (Shane West) begins treatment on a woman who soon requests a new doctor.
