Susan meets with Kerry about her one-month performance review, but before they can start Susan gets a phone call from Chuck. Susan hastily lets Chuck go and Kerry tells her that her overall performance is good and there is only minor glitches she needs to work on. It sounds to Susan that Kerry is being chastising but Kerry tells Susan that she wants her to take more leadership.  John looks at pictures of Kem on the computer, which she had sent to him. Jerry looks over John’s shoulder telling him she looks great, but realizes his statement and rephrases it. John agrees as Ray asks John if he should know her. John tells him he’s not her type.  EMT’s bring in a patient, which Ray and Carter take. The begin treating a 60-year old woman with facial trauma, chest contusions and injuries to both her arms. They ask her name but due to the injuries on her face she is unable to speak. One of the paramedics says he thinks her name is ‘Louise’. While reviewing Louise’s x-rays they clear her neck to get an airway going. A police officer wants to question Louise, but John doesn’t think it’s a good idea at the moment.  Sam and Luka arrive and John presents Louise to them. John gets called away to another patient and Ray prepares to intubate. Luka announces that they can’t intubate Louise due to her jaw being broken. They decide to insert a tube into her throat to help her breath, but as Sam is suctioning her mouth, Louise manages to get out that she was raped. 

Sam stays with Louise when the police officer returns. Sam walks him out of the trauma room and gives him the rape kit they did on Louise. The police officer tells Sam that they had found signs of struggle in Louise’s apartment, but there were no witness’s. The police officer tells her that she most likely fell from the fire escape trying to outrun her attacker. Sam asks about family and he replies they hadn’t found them yet. He tells her that some detectives were coming in to talk with Louise, Luka and Sam. Susan talks with the interns on the patient wait time. Neela announces she has a suture patient waiting and leaves. Haleh comes to tell Abby something about her patient and she leaves. Ray dismisses himself to go check on some lab results. Abby meets a patient by the name of Mi Cha Pak who is complaining of stomach-ache.  Mi Cha tells Abby that it comes and goes. Mi Cha tells her that she thinks it might be kidney stones because her father had them. She tells Abby that she is all better and wants to leave. Abby talks her into staying so they can do some tests to be sure. A detective comes in to talk to Sam about Louise. Sam tells him that they will be moving her up to ICU soon. Sam introduces Louise to the detective and tells him she can’t talk because of the tube in her neck. The detective asks Louise if she knew her attacker. She shakes her head. Louise starts coughing so Sam suctions her tube. Sam tells the detective that is all she will allow and to speak with Luka on when she will be able to talk.

Abby continues to treat Mi Cha. She does a pelvic to which Mi Cha hollers in pain. Abby asks about her last period, and if she has had a gush of water. Abby discovers that her patient is pregnant, and fully dilated. Haleh calls OB and Abby tells Mi Cha not to push.  As they make their way to OB Chuny asks if she is pregnant and Mi Cha tells her no. Luka tells the detective that Louise is in critical condition and it still could get worse. The detective still wants to talk to Louise but Luka tells him he didn’t think it would happen for a while. The detective tells Luka that he thinks Louise’s rapist is the same guy that has been raping women all over the north side. Abby and John wheel Mi Cha to the elevator as Abby explains her patient to John. Abby realizes they won’t make it up there in time and John offers the delivery to her. Abby gives it to John. John delivers the baby and tells her that it’s a healthy baby boy. She stares at the baby and tells John that it isn’t her baby. Abby tries to break through Mi Cha’s denial that she has just given birth. Haleh asks John to help Abby, but he suggests they call psych. Haleh pushes John and he goes to talk with Abby. 

Luka notices the detective on a pay phone. He goes to ask Sam why he is waiting. Sam tells Luka that she believes Louise wants to help the detective find the rapist. Sam says Louise can whisper if they deflate the crich balloon. Luka tells her that it is possible but it would jeopardize Louise’s already critical health. Luka tells her she is too soft with cases like Louise’s because of what she went through with Steve. Luka gets called away to another patient. Mi Cha’s mother arrives and is sent to the waiting room. John goes in to see Mi Cha and she tells him that she has to go and that her parents get upset with her if she doesn’t go home right after school. John tells her they are still waiting for some test results. He asks her if she has seen the baby, who is rooming in with her. Carter picks up the baby and asks Mi Cha if she would like to hold him. Mi Cha asks why and asks if she will get to go home soon. John smiles and tells her ‘I hope so’. Outside the room Abby asks John if Mi Cha is pretending to be confused and he doesn’t know. Abby tells John she cannot break patient confidentiality and tell the mother. John sees Mrs. Pak walking towards them. He tells Abby to call Wendall and psych. He greets the mother.  

Neela notices EMT’s bringing in a girl scout with a head bandage. The paramedic describes her as a 43-year old. Ray meets another EMT who is wheels in another girl scout on a cell phone. They tell him she is a 28-year old male with penetrating wounds. Ray catches on right away that it was a drill. Morris calls for Susan.  The detective tells Sam that they got some background on Louise. He also tells her that the rapist uses the same MO. He poses as a pizza man who is lost in the apartment building of his victims. He tells Sam also that 1 of the victims has already died, but each of the other victims has brought them one step closer to catching the rapist. Susan reminds everyone that response times are being tracked. Haleh tells John that Wendall has arrived. Luka tells Susan that the drill is crazy, and Ray comes to tell Susan that he wants a real patient to treat. Wendall talks with Mi Cha about her high school. Mi Cha tells them that she has to be home right after school to care for her brother, who is 23. Wendall asks her what they do and she ignores the question stating she’s tired. They leave so Mi Cha can rest. Abby tells Wendall the mother is still waiting and she says she will go talk to her. Wendall tells them both that the child can go into foster care. John is angry with this because there is a mother and grandmother present. John marches into the family room to ask Mrs. Pak if she ever noticed any changes in Mi Cha, which she hadn’t and wanted to know what was wrong with her daughter. John asks her if she knew her daughter was pregnant. John tells Mrs. Pak that her daughter gave birth to a healthy baby boy that morning. Mrs Pak tells them her husband will disapprove and when Mi Cha can go home. John tells her it will be at least a couple of days. Mrs. Pak out anxiously. 

Chuny tells Sam that Louise is awake. Chuny leaves to help with the drill. Sam tells Louise about the rapist and how many victims there have been. Sam tells Louise that she can help her talk to the detective by deflating the crich balloon so that she can whisper. She tells her it is risky because her lungs would be oxygen deprived but assures her that if there is any trouble they can just re-inflate the balloon and get the oxygen back into her lungs. Louise understands and Sam tells them they need Luka’s approval. Louise nods. Sam goes to find Luka and asks Frank to alert her when he comes back. The detective walks by and tells Sam he is leaving for a while. Sam tells him that Louise is awake, but he tells her that he will talk with her later, because she can’t talk. Sam tells him there is something else they can try. 

Kerry is upset that Susan didn’t warn the staff about the drill. One Girl Scout offers to help with the ‘patients’ but Susan tells her to go back to her area. Susan almost gets run over by a group of screaming wet girl scouts. A soaking wet Morris chases them saying he was mocking a decontamination shower that turned into a real one. Sam deflates the balloon and Louise whispers that she is ready. Luka returns to the ER and Frank tells him that Sam was looking for him. He discusses a patient with Chuny as he walks to where Louise is and just about enters the room when he decides to check in on the patient Chuny and him were discussing. The detective asks Louise about her rapist and she tells him he was white. She starts coughing and Sam questions that it maybe too much. Louise continues saying he was medium height.  

Abby tells John that Mi Cha’s brother is on his way. John acts unaware of the case after what happened with Mrs. Pak. He looks for a form and finds prints of Kem’s pictures. He asks who printed them out and Frank replied that he saw Morris looking at them. John goes after Morris. He finds him in an elevator with some girl scouts. He asks what he was doing printing out pictures of Kem, but the doors close before John gets his answer. John walks back into Mi Cha’s room and goes to the baby. He tries to get her to hold the baby and she panics asking him why he keeps trying to push the baby on her. Wendall sees this and calls him out.  Louise and the detective are still talking. She starts coughing and vomits. Sam asks the detective to find Luka. Luka comes in and Sam tells him she vomited and Luka tells her she aspirated the vomit into her lungs. Luka tells the detective there will be no more questions. Luka lectures Sam on the procedure. Sam tells him she didn’t expect Louise to vomit, and she was just trying to help because it was what Louise wanted. Luka tells her it’s what she wanted and asked if anyone else knew what she did. Sam replies just the detective. Chuny calls them back in because Louise’s vitals dropped. 

Abby tells John that Mi Cha’s brother has arrived. Again John pretends he is not involved. Abby informs John that the brother is developmentally delayed. Abby also tells him that Wendall is in with them now and asks him if he would like to join them. He tells her he is not interested. Abby introduces herself to the brother and Wendall proceeds to ask him what they do when they get home from school. He tells her that Mi Cha never goes out, never leaves him alone. Mi Cha asks her brother what he is doing and he tells her that he didn’t tell anyone. Mi Cha asks why he came and he told his sister he wanted to see the baby. She gets out of bed, and her brother starts crying telling her that he didn’t tell anyone and he didn’t do anything wrong. Haleh and Wendall lead him out. 

Susan battles with fake patients. One Girl Scout begins to list off all of her badges and Susan gets frustrated telling everyone to get out, including the drill instructors. Pratt sees Deb leaving with her father. She tells him he has been in County with pneumonia for the past 3 weeks, and that she thinks he is better off at home. Abby and John watch Mi Cha, and her brother with the baby. John retells that Mi Cha had a boyfriend across the street, and used her brother as a cover. He walks off as Wendall enters and asks why John didn’t come in. Abby tells her all about Kem and the baby. Deb and Pratt walk out with her father and as he gets more and more agitated, he rips out his feeding tube. Deb curses. Louise crashes and Luka curses. Sam starts compressions. They try to revive her with the defibrillator but nothing happens. Pratt tells Deb that her father is better off in a nursing home and Deb tells him she could never do that to her father. Deb inserts the feeding tube back into her father’s stomach. 

Susan comes into the trauma room where Sam and Luka are trying to revive Louise. Susan tells him that aspiration doesn’t result in cardiac arrest. Luka tells her he thinks the crich balloon burst. Sam continues to do compressions, but Luka gives up and tells her to hold them. He calls Louise’s time of death. Sam states she will clean Louise up. Luka tells it’s a coroner’s case, but Sam tells him they will see that the balloon never burst. Luka takes a syringe and injects air into the balloon, saying that they will see that it did. Sam watches him leave.

John sees Mi Cha holding her baby. Wendall joins him and he asks her if she will keep him. Wendall responds that she is holding him now and that’s a step. He tells her about Kem losing the baby and she tells him she heard. She asks if he is getting help and he responds that he is trying. Sam gets Louise ready for the morgue. Luka comes back and tells her she doesn’t need to do this job. Sam cries over her Louise stating she should have never done the procedure. Luka tells her she could’ve died anyways due to her injuries, but Sam continued saying that the detective asked her too many questions and all Louise could remember was that he was a ‘white guy with dark hair’, Luka caresses her and she repeats ‘ I did what I did for white guy, dark hair’ Luka takes her in an embrace as Sam cries.

RAPE VICTIM TRIES RISKY PROCEDURE TO HELP POLICE -- Sam (Linda Cardellini) and Kovac (Goran Visjnic) treat a woman who was brutally beaten and raped and wants to help police although she can barely speak. Lewis (Sherry Stringfield) schedules a JCAHO emergency drill without telling the staff. Carter (Noah Wyle) and Abby (Maura Tierney) treat a girl complaining of kidney problems, which is just the beginning of her adventure. Laura Innes, Mekhi Phifer, Parminder Nagra and Shane West also star.
