The episode begins with carter and Kem in their living room going through baby shower gifts and carter putting paint samples on the wall presumably for the nursery. They then discuss naming the baby George and where they want the baby to be born. Carter puts his head on Kem's stomach and says to his unborn son "Kick once for America. twice for the mud hut." Then we cut to Sam doing laundry and pulling stuff out of Alex's pockets. She comes upon the last thing she expected to find in her ten year old's pants, a condom. She storms into the kitchen to confront Steve who just took Alex to school, asking him "what's this?" Steve responds with a smart comment "honey you knew what that was when i met you at fifteen." Apparently she didnt seeing as she had Alex at that age. Anyway she wonders where he got it and Steve says he picked up a few just in case and that Alex had found it. Then he tries to kiss her but she moves away and asks him to watch Alex tonight so she can go out. He asks her if its a date and Sam remains silent.

Neela, Pratt, and Abby are standing at the Admit board discussing graduation. Pratt makes a comment about them being incompetent baby doctors and Morris says they should wear Caution signs. Personally i think he is the one who needs the sign. Neela tells Abby and Lester that she has nineteen relatives flying in, Abby reveals that she has none. Pratt then tells them to get to work and learn some real medicine before they start getting malpractice lawsuits. The three med students have a few choice words for him as the credits begin.

Sam and Luka are outside waiting on an ambulance discussing their plans for tonight. Luka wonders if Alex is coming but Sam says it will just be them because she misses him. Luka grabs her hand as the paramedics arrive with a 80 year old man who was wandering around naked at a nursing home. Sam and Luka grin at each other and take the man into a trauma room as Frank returns to work saying his wife was sick of him being at home.

Meanwhile Carter wakes a sleepy Kem and gives her orange juice and asks if the baby has been kicking. Kem says he never stops. At the hospital Neela is questioning Pratt about Elgin, the seventeen year old from "Abby Normal" who burned his hands on a grill but who had extremely high blood pressure. Neela is worried that the teen has CHF (Congestive Heart Failure) but Pratt tells her that is impossible and to call and make sure that Elgin goes to the clinic sometime before he's thirty. Abby and Luka are trying to figure out why Ferguson, the naked man was wandering and has dementia. Luka asks Abby if she got her boards results yet and if she is going to graduation. Abby says no but they will let her walk without getting a diploma. Morris interrupts the conversation.

Rachel Greene shows up looking for Dr. Corday. Frank directs her to the sixth floor. Carter is speeding to the hospital with Kem after realizing that something is wrong with their son. At Admit, Neela hangs up with Elgin as Sam does charts in the background. A man walks in with a box looking for Samantha Taggart. Sam says unless he is a process server then thats her. Inside the box are ice cream sundaes from Baskin Robbins. Sam knows who they are from and half-smiles.

Upstairs Elizabeth is on the phone with Jenn, Rachel's mom. She hangs up and goes out to where Rachel and her bf BJ are. Elizabeth and Rachel are talking when Dr. Lawson interrupts about plans he and Elizabeth have for the weekend. Rachel tells Elizabeth that she is looking at colleges in the area and that she wants to stay with her and Ella for a few days. Elizabeth agrees but says the teens have to have seperate rooms.

Back in the ER, Sam, Neela and Abby are eating ice cream and Sam is telling how she met Steve when she was fifteen working at Baskin Robbins. He was 23 and a construction worker. Then Neela asks if Sam's parents had anything to say, Sam says they had plenty but she didnt listen and continued dating Steve the end result being Alex.

Kem and Carter are with Vicki having a ultrasound. Carter's voice is cracking as he realizes that his son is dead. He then grabs Kem's hand as the doctor confirms his diagnosis. Labor has to be induced now for the baby to come out. Carter and Kem are in shock.

Neela is still trying to get Pratt to help Elgin. She wants to make a house call by herself in the bad part of Chicago, Pratt tells her he will go with her if she buys him food. Weaver wants the naked man put in restraints. Frank tells Luka that security called and that his car alarm is going crazy in the garage. Morris asks Neela out as Weaver overhears him tell Neela that Abby failed the boards.

Kem is getting settled in upstairs in delivery as Luka walks out to the garage and sees Steve leaning against his car. Steve basically tells Luka to return the family sized SUV because Sam and Alex belong with him, not Luka. Luka stands there unsure what to say or do.Pratt and Neela are at Elgin's looking for the teen. They find him and he promises to return for a checkup. Luka asks Abby if she is going to her graduation and then Weaver pulls Abby away and turns on the whole Chief of Staff routine. She gives Abby a hypothetical "if someone who fails the boards was on County's match list then that leaves County short an intern for the next year." Abby responds with a" hypothetically this person could report the Chief of Staff in violation of clear rules on how matches are to be conducted."

Sam bounces out of the hospital with anticipation of her date with Luka. Luka tells her that they need to take a raincheck and Sam immediately wonders whats up. He tells her that it confuses Alex, Sam says no it doesn't but Luka wont listen. He offers to give her a ride home but Sam walks off hurt towards the EL.

Carter sits with Kem upstairs looking out the window. She tells Carter not to tell anyone what is going on. Elizabeth and Rachel are drying dishes talking. Elizabeth is floored to hear the real reason that the teen is back in town, she needs a prescription for Plan B (the morning after pill). Back at County, Carter goes for coffee and talks with a man whose wife is giving birth to their third child.

Sam is washing her face in the bathroom as a drunken Steve appears in the doorway behind her. He tells her she is beautiful and Sam walks out of the bathroom. She questions him about going to the hospital to talk to Luka, he doesnt respond and Sam tells him to stay away from the hospital. He blocks her from going into her room and tells her that he loves her and he wants another chance. He tries to kiss her, Sam kisses back for a split second, but then pulls away and says "Steve please". He continues to try to seduce her and she repeats "Please Steve." trying to get him to stop. He hold the back of her neck with both hands roughly and loses the sweet tone "Please Steve what?!" and Sam tells him to let her by. She then shuts her bedroom door and leans against it in tears.

Anyway its now four something in the morning and Carter's son is born. He walks out of the delivery room in tears and collapses in his father's arms.

7am- Weaver, Chen and Corday are talking about Kem delivering a stillborn baby. Neela is showing her family around the ER, giving them the full tour. A nurse upstairs asks Carter if he wants a lock of his son's hair. He tearfully cuts it. Elizabeth asks Lawson to write Rachel's script for her, Rachel later wonders why Corday didnt write it herself. Elizabeth says because if Jenn saw Corday's name on it she would hit the roof.

Abby and Luka are sitting with Carter outside in the lobby. Carter tells them that he appreciates them being there but that they need to get to graduation. Abby kisses Carter on the head and tells him to let her know if he needs anything. Luka shakes his hand. Carter's dad stays with his son.

At graduation, Luka sits with Susan and sees Sam and Alex sitting with Steve. Susan is a week late for delivery. Elgin is brought in the ER with pulmonary edema and CHF. Abby runs into graduation just as her name is called. Luka, Susan, Sam and Alex cheer loudly for her.

Carter examines the lock of his son's hair as he tries to figure out how to convince Kem to hold their son. Luka and Susan ask Abby to lunch, she politely declines and Luka sees Sam and Alex talking with Steve. They dont see him. Abby calls her mom and tells her that she graduated medical school. Carter tells Kem that they have to say goodbye to their son and they need to hold him. He tells her he loves her and that he wants to spend the rest of his life with her. They are both in tears as Carter puts the baby in Kem's arms and the screen goes black.
