When Carter wakes up in the morning Kem is not in the bed. He seems worried. He goes downstairs and his maid Emily says she's in the kitchen and again has offered to make breakfast for her. She tells Carter to tell Kem to stop doing this, but carter says she's just trying to be nice. He proceeds to the kitchen to say hello to Kem and tells her that Emily his maid feels "her job is being threatened", but he's just joking around with her. Carter tells Kem that soon she'll be walking around the house in just her socks. Kem wants to know if this isn't going to upset anyone and Carter says no, because she's already one of the family. So Kem grabs the socks he was wearing, puts them on and sets off running to the bedroom, Carter following her.

Back in the hospital, Kerry says that things are pretty much settled down so she allows Gallant to leave earlier. When he gets home, Pratt is with his sister Valerie. They argue, and when Pratt leaves, Val confesses that he's already been around a few times. Gallant seems very pissed off that Pratt was with his sister.

Meanwhile, Luka informs Kerry that he is going back to Africa and his last shift is January 18th. Weaver wants him to put it in writing and leave it in her office a.s.a.p. A while after Kerry has already left, Malik and some nurses tell Abby to talk to Weaver about the ending time of their shifts. They aren't allowed to leave until they find a missing valium pill. Abby calls Weaver and unexpectedly finds the valium in Weaver's lab coat. She tells her that they have found it and hangs up. The nurses leave, but Abby has to stay.

Mrs. Webster a patient  comes in suffering from dementia; Luka takes her as his patient, while Pratt develops some kind of affection towards her when she thinks he is her son.

Carter and Kem go to the hospital and they're received by Sam, who didn't know him yet, so she thought he was another patient. When he introduced himself, she said that it was nice to finally "put a face to the name". When they get in, Malik, Frank, and the nurses walk over to greet him, and they meet Kem. Carter then announces that they're having a baby. Everyone is excited and congratulate Carter. Carter meets Morris and Neela while they're doing rounds with Pratt, and there's an incoming trauma that they have to work in together. The victims are Mr. Brodie and his son Martin, who has a spinal weakness that seems to make the case even more difficult. Carter and Abby keep avoiding each other unexpectedly, although they don't really mean to. Carter goes up to the OB with Kem and finds out that the baby is a boy. Abby sees him at Admit when he comes back from OB, and she approaches to say hi. She meets Kem for the first time, and things are pretty awkward between them. Its eating her up that he is with another girl. Abby tells Carter that she's back in MED school, and in return, he gives her the news that he's having a son.  Abby sees the ultrasound and congratulates them. Then Luka walks by and meets Kem for the first time as well, she says that if it wasn't for him, she and Carter would have never met at all. Carter says that he has to get back to work so he and Kem leave. Abby asks Luka did Carter "knock her up the minute he got there", and Luka just shrugs. He asks if she's going home, but she says she has to be there, so they both get back to work.

Pratt finds Mrs. Webster out of bed, and he takes care of her, afraid that something might happen to her if she is walking around the hospital such a mess. The lady says Pratt is a good boy, and he jokingly answers that most women tell him that.

Mrs. Hurlsey comes in with migraine, and Carter takes her along with Morris and Lester. She asks him if there's any chance that her children could be fed there, and when she mentions the food both students throw up. Carter says that they should be working, so they apologize and leave. The woman says that even a sandwich would be enough because they are not picky, and Carter discovers that they haven't eaten enough in a long time. He admits the kids at Peds and takes care of the mother.

Elsewhere, Mr. Brodie needs a scan, and as Pratt is about to do it his pressure goes down. They page Corday, but Pratt decides to do the scan anyway because he has no time. When Corday gets there, everything is fine, and she then knows where Mr. Brodie's problem is exactly.

Brodie's son is hypoxic, and he is having trouble to breathe. More over, Neela is alone and all doctors are busy. Some minutes later Pratt comes along and decides to intubate the boy. Neela asks him if he's sure about it and he says yes, that the boy needs oxygen as soon as possible. Carter comes in just in time to see Pratt perform the intubation and accidentally break the kid's neck in the process. Pratt gets out of the room, and Mrs. Webster (who is walking around again) tells him "it is ok" when she sees that he's a bit down. He tells her to go back to bed, and orders a nurse to take her back to her room. Before leaving, Mrs. Webster says "but momma still loves you", and leaves Pratt thinking. However, there's another trauma coming in and he has to work side by side with Gallant. As they work, Pratt asks about Val, but Gallant refuses to say anything to Pratt about his sister.   The woman they're working on has cancer, and Luka tells her that she should go up to the OR for surgery. She tells him that it's too late to save her and that they'd better save the money for another patient who's worth it.  Alex sutured himself up after cutting his leg, and Sam asks Carter to check on the wound. Carter fixes the problem and leaves. Sam goes back to Admit, where she finds Abby looking for some charts. She asks if her shift has already started but Abby says that since she couldn't sleep she decided to "make herself useful". Sam asks Abby if the insurance policy covers psychological therapy, and Abby says she thinks most of it, but there's an annual maximum of interviews.  Val shows up at the hospital, and Gallant thinks she's there to see Pratt, but she reassures him by saying that she didn't want to leave things like that with him after what she'd done. She tells Gallant that she'll be more considerate the next time, and he says yes, then she tells Gallant not to be so serious all the time.

Corday gets back from the OR and tells Pratt that Mr. Brodie's aneurism has been taken care of, and he's in recovery. When she asks about his son, Pratt sadly tells her that he accidentally broke his neck. He looks out of the window thoughtfully, and spots Val getting into a cab so he runs down to catch her before she leaves. When they're finally together, he tells her he has four days off the next week and suggests to go and visit her in Washington. However, Val refuses and says that although she's had a lot of fun, this has been no more than a vacation for her. Pratt is fairly surprised and hurt at Val's comment. Meanwhile, Sam and Luka seem to be getting along better when Sam confides in him some of her issues. She tells him how hard it is to be a single mom and that she's afraid of doing everything on her own and taking responsibility for everything. *wink*wink* (is something happening here) Luka tells her she'll be fine.

Pratt's shift doesn't get any better when he finds out that Mrs. Webster is dead, so he goes to see her one last time before the people from the morgue take her. Right after, he accepts the hard challenge of telling Martin Brodie that there was a problem in his spine and he'll probably end up paralyzed. Pratt's day couldn't suck more.  After a trauma, Carter and Abby stay in the room, making small talk. He tells her he is glad she's back at MED school, and she says she is too. Then he says she seems happy, and Abby answers with a friendly "right back at ya". When Carter asks if their friendship is okay, she says yes, but when he asks her to join him and Kem for dinner, she laughs and says that they're "not that ok". When Carter leaves the ER, he finds Kem waiting for him outside. She tells him she's never seen snow since she was a kid, and then asks him if he arranged for it to snow. Carter says yes and asks if it was too much, but she says no.

Alex had fallen asleep in the lounge, so Luka told Sam to move the car into the ambulance bay and he would carry Alex to the car. Sam accepts the offer and thanks him. Once they're outside, she offers Luka a lift, but he says he has to do something first. Luka breaks into Kerry's office to postpone the day of his last shift, and Kerry finds him. She asks him why he was in her office and he tells her about how he wants to postpone the date. She asks if he's leaving sooner, but Luka surprises her and says he's going to leave later, but is not sure when. (hmm..I wonder why...) Kerry tells him to inform her when he's sure about it.

Pratt is having a beer at Ike's, when Gallant comes in and starts talking to him. Pratt tells him he's not in the mood, and so Gallant tells him that his reaction was the result of her sister being the only girl in the family. He says that he and his brothers have always been very overprotective with Val. Pratt says that Valerie played him, and Gallant tells him not to say that he didn't warn him. Then he grabs his things and goes out of the bar, leaving Pratt to his thoughts of the day and Val.

NBC Review

DR. CARTER RETURNS FROM AFRICA FULL OF SURPRISES INCLUDING A NEW LOVE; THANDIE NEWTON GUEST-STARS -- The return of Dr. Carter (Noah Wyle) from Africa prompts various reactions, especially when he is accompanied by his new love Makemba (guest star Thandie Newton, "Mission: Impossible 2") -- and Carter has other surprises in store -- while Dr. Pratt (Mekhi Phifer) squares off against Dr. Gallant (Sharif Atkins) over a personal matter. A busy Pratt also finds time to treat a young victim of a car accident whose case is complicated by a rare form of osteoporosis and takes an interest in an elderly woman suffering from dementia who believes he is her son. In addition, a whole nursing shift cannot go home until they find one missing valium pill. Elsewhere, Dr. Kovac (Goran Visnjic) informs Dr. Weaver (Laura Innes) about his departure date and Carter deals with a malnourished woman and her equally famished family. Alex Kingston, Maura Tierney, Parminder Nagra and Linda Cardellini also star.
