This episode starts when we hear African music, on an El train. The camera swoops past people, and then focus's on Dr. Luka Kovac. Luka is looking at an add. The camera then goes to the next scene, where we see Luka in County General, back from Africa. When Luka comes in, Chuny greets him with her smile. He just smiles back, and he says something. Frank looks at Luka, and tells him to 'Call Weaver.' As soon as Malarkey hears Luka's name, he jumps over and introduces himself to Luka. As Luka walks on, a lady brings her baby up to him, asking him for help. Apparently, she did not strap her child in the cart good enough, and so Luka takes him off to trauma.

Chuny and Dr. Pratt are staring at this guy, who is ranting about evil messengers. One of the security guards wants to spray pepper spray into the room, but Pratt tells them it would cause mayhem in other parts of the hospital. So, Dr. Nick Cooper and Dr. Pratt go into the room with a gurney, and strap the guy down, do Malarkey can give him a nice shot of calming Haldol. But, Malarkey is busy doing something else, so he doesn't do it. Finally, a new nurse, by the name of Samantha Taggart, walks in, grabs the needle, and gives it to the guy. After she's done, Romano walks by, and questions her about if she works there. She politely introduces herself to him, and tells him she is a new nurse. Next, an African-American woman taps on the window, and asks Sam how much longer the wait is going to be. Sam answers her, but the lady does not understand. Romano starts talking out loud about which attending is going to be his daily rage. In the trauma room, Luka is taking care of the baby, when Malarkey walks in, and tells Luka Weaver is waiting for him outside, in the hall. He leaves Malarkey in charge, promises the mother they will get a head CT, and goes outside to talk to Weaver. Romano is now demonstrating his new mechanical arm, and Pratt is calling him RoboDoc. As Romano turns, his rubber fingers brake some glass. It is pointed out to Malarkey that the baby's stats are dropping, and then he notices it. He wants to intubate, and Chuny runs out and tells Luka about it. Luka comes in, and tells Malarkey STOP twice, the second time, forceful. He explains that the correct procedure is to bag the baby. Susan leads the mother and the other child to the waiting room, where the other child goes to get water for the mother. While he is away, Susan is questioning the mother, on if she has been drinking or not. The mother replies no, but Susan tells the mother that she can smell is on her breath. She wants the mother to take a blood-alcohol test, when the mother admits she made a little mistake. Next, you see Ben Hollander (played by Bob Newhart) get wheeled into the ER. His neighbors called 911 when he didn't answer the door. Ben is crabby, and makes some comments that he wants to get out of there. A nurse tells Susan his levels are weird, so Susan hooks him up to an IV, for fluids. She tells Ben it's almost like Gatorade. Next, Susan is outside telling Luka about the mother, and about how she is drunk. Luka tries to tell Susan he doesn't think so. Susan is worried the kids will come back tomorrow with worse injuries, when Luka acts like he doesn't care. Next, you see Neela presenting a case to Luka, when Romano walks by and questions if that is Susan and Pratt working on a case together, but Neela tells him he is looking at Gallant. Luka tells Neela to just discharge the patient, and call him if labs come back abnormal. You then see Abby in the background, and Romano says a few words to her. Abby and Susan go out to Lunch, and talk about Abby in med. school. Abby is going to med. school to become a doctor.

Romano comes in, and asks Abby a question about surgery, and Abby answers him. He then turns to go to the phone, to call Elizabeth. The patient is a surgical candidate. They call for a nurse, and they want Abby. Abby rolls her eyes and walks away, as Samantha Taggart comes into the room to help. The next scene, you see Henry, the little baby's brother, whispering that the baby is going to be ok. The mother is standing beside him, when Susan brings the news. She tells the mother it looks good, but only time will tell. Romano is now trying to rush Luka and Abby through things, and is complaining that the surgery wait is taking too long. Luka is attracted to a woman named Athena. Kerry Weaver and Luka have to talk about Luka’s schedule, since he is now back from Africa. Luka lets Weaver know that in two months, he is going back to Africa, when Weaver complains to him, for not letting her know 2 weeks earlier, last time he went. The woman from earlier, the baby's mother, has come back to the hospital, claiming that Henry burnt his hand. Her husband was supposed to pick the kids up, but didn't do that. Susan calls Social Health Services down, and a lady tells Susan that they are going to place the kids in the hands of their father, because the mother has a severe alcohol problem. The mother tells Susan she doesn't want that, and Susan assures the mother that its only until she can get back on her feet again. Neela is sitting with Henry, and is making conversation with him. The husband arrives, gets filled in, makes the mother mad with a question why its never her fault, and then the mother leaves. Meanwhile, Corday and Diego are in surgery together.

The next scene shows Ben Hollander again, using a huge magnifying glass to read the paper. When Susan Lewis asks him about it, he admits he has a macular degeneration. Sadly, none of the new treatments would help his disease. Susan tells Ben that is he does not get help for his disease, she will call psychiatric services down for three days for him.

Malarkey and Cooper are outside talking to each other, when the mother of the baby, drives up in a SUV, pours gas all over herself, and then strikes a match after Henry calls her name. Luka, Cooper, and Susan all run to get a gurney, run outside, get a blanket, shatter the window, and try to save her. They get her out, and Susan asks her if she can hear her. Over ninety percent of her body is burned, and they roll her into the ER, and they give her morphine. She has a ten percent chance of living. Dr. Diego walks up to Jacy, and asks her where Corday is.

Next, Corday is getting mad at the coffee machine. Its broken, and she cant seem to use it. Susan walks into a trauma room, where the father and Henry are sitting down. Henry has his head in his dads lap. His dad had no idea that she would do anything like this. Malarkey and Cooper are gossiping about the mother at the front desk. Pratt makes fun of Romano for trying to do something with his robot arm. So, Romano makes Pratt take over.

Ben is now complaining to Chuny that he is waiting too long, so out of frustration, he unplugs himself and sneaks out of the hospital. Luka and Sam are now wheeling a woman strapped to a gurney who's left pupil is blown, into a room. Luka notices the woman was there, but hasn't been treated. He figures out Malarkey was treating her. Her name is Athena. Her brain is massively bleeding. The scene shows Paula in the hospital room. She wants to touch the baby Adam, but doesn't get to, because he pulls his hand away. She asks about Henry, but Susan tells her that he’s pretty upset and doesn't want to come in. Athena, the woman Luka, Coop, and Malarkey were working on dies. Luka blames it on Malarkey for him not moving faster. When Malarkey walks out the room, a guy is screaming to get out of there. Malarkey runs him outside of the glass doors, and then screams he quits. Pratt walks by and makes a comment, but Coop tries telling him that Pratt was just teasing. Susan is worried about Ben. She knows he left, and she tries to reach him at home, by the doesn't pick up the phone. So, she and the ambulance crew go into the ambulance to go to Ben's house. Before leaving, she tells Neela to page her is anything happens with the mother. When Susan and the ambulance crew break into his door, Ben looks up and asks if there is a problem. Samantha is sitting outside of County General Hospital staring into the sky, when Abby and Elizabeth approach. Abby introduces Elizabeth to Samantha. Luka rounds up Coop, Gallant, and Neela, wanting to let them know that they need to treat three patients in an amount of time, so that everyone will have seen the patients by ten o'clock. Ben and Susan are talking, and Ben is complaining to Susan that he had waited three hours and no one came to see him. Susan and him talk about other things, too. We go to the scene where Luka is watching Coop, Gallant, and Neela, and then Romano walks by, and wants to know what they are doing. Luka tells him" Providing better triage." We then fade to black as we see Luka’s pleased face.

DR. LEWIS FEARS FOR ARCHITECT WHO'S LOSING EYESIGHT; EMMY WINNER BOB NEWHART GUEST-STARS -- Dr. Lewis (Sherry Stringfield) fears for a distraught architect (Emmy Award-winning guest star Bob Newhart, "The Bob Newhart Show") whose degenerating eyesight makes him despondent while a spirited new nurse (new series regular Linda Cardellini) replaces Abby (Maura Tierney) -- who decides to return to medical school. Elsewhere: disaster strikes when the doctors suspect a mother (guest star Allison Elliot) is abusing her child; Dr. Corday (Alex Kingston) learns something shocking about the charismatic Dr. Dorset (guest star Bruno Campos, "Jesse"), and a green medical student (guest star Scott Grimes) is overwhelmed by the chaos and grisly conditions of life in the E.R. Laura Innes, Paul McCrane, Sharif Atkins, Mekhi Phifer and Parminder Nagra also star.

October 30, 2003: Out of Africa: Bob Newhart begins a three-episode role as an ailing architect whose gradual loss of vision touches a nerve in Lewis. Meanwhile, the rest of County is thrown off by a staggering backlog of patients; an alcoholic mother's grisly reaction to losing custody of her children; the arrival of a new nurse (Linda Cardellini); and Abby's career switch.
