Carter arrives home from Africa and heads to Abby's apartment.  He goes inside and sits down beside her as she is lying asleep in her bed.  He gently kisses her on her forehead and she wakes up.  He tells her hi and she says hi back.  "When did you get back?", she asks him.  He tells her just now.  She reaches over and pulls out a pack of cigarettes from the nightstand and tells him that it is 5:30 in the morning.  His flight from London was delayed.  He tells her that he missed her.  "Did you just let yourself in?", she asks him.  "I'm sorry about how I left", he explains.  Can I have my key back please?", she replies.  She lights up her cigarette and Carter takes the keys off of the key ring and tosses them in a bowl on his way out.

A woman is bringing her daughter named Hannah out to meet the carpool.  She gets in the car with Katie and Matthew.  Pratt rushes toward work.  The woman driving the carpool gets stuck in traffic.  Pratt just makes it on the train.  "if I'm late again my boss is going to kill me", he tells the young woman sitting next to him.  "You're a doctor?", she asks him.  He looks at her and replies, "Yeah, yes I am" and gives her a big smile.  Katie asks her mother where they are going and then the cell phone rings.  The woman answers, "Denise Johnson", and then passes out.  The kids in the back begin to yell as the car keeps moving, knocking off a car's door and then going upside a car and flipping over.  The car lands on it's roof and Denise begins to come to, just in time to see a big truck coming right at her.

The train has to stop because of the accident, causing Pratt to be late.  Gallant has graduated from med. school and was fourth in his class.  A lot of construction is going on and it has Kerry upset, causing her to yell and scream at the construction workers.  The who waiting area is in disarray.  A young woman arrives and introduces herself to Frank as Neela Rosgotra.  Frank walks away and asks, "Don't we hire Americans anymore"?  Abby searches for Susan to assist with the woman and three kids.  She locates her in the Unisex bathroom with her ex-husband.  She tells her that the MVA has almost got the people out and there will be three adults and three kids being brought in.  She tells Susan that she thought she was divorced and Susan replies, "We are".  Abby then tells her that Carter is back.  She then tells him that he tried to apologize.  Kerry continues to argue with the workers.  Frank tells Dr. Lewis that she has another TWA.  Neela introduces herself as a third-year medical student and Susan tells her that she may want to wear something different next time.  Neela is dressed nicely.  The paramedics begin to bring in the accident victims.  The computers go down and Kerry gets her eyes full of 'plumber's crack".  Pratt arrives and sees that Carter is back.  Kerry gets on to Pratt for being late.  Carter walks in and asks Frank where the doctor's lounge is.  Kerry asks him if he brought Kovac back with him and he tells her nope.  Carter walks in to the lounge and finds his things in a bag.  Abby has emptied his belongings from her apartment and left them for him there.

Dr. Gallant assists Matthew who has a hurt arm.  Pratt asks Haleh who the young woman is in the next room with Dr. Lewis.  Haleh tells him that it is a new med. student.  Pratt doesn't wait too long before he goes over to check her out.  Pratt introduces himself to her and starts the flirting.  Susan is with Neela and Pratt.  They are working on Denise, the mother who was driving the car.  Frank tells Carter that an attorney called everyday that he was gone and that he wants him to call him back.  He is the executer of his grandmother's estate.  Carter goes in to see if Susan and Pratt need any help.  They are doing okay, so he goes in to check on Dr. Chen's and Abby's young patient.  Jing-Mei excuses herself so that he and Abby can catch up.  Abby and Carter begin to disagree on the patient's care.  Jing-Mei walks over to assist Pratt and Susan and sees that Pratt is getting friendly with the new med. student.  Pratt introduces Jing-Mei to her.  Carter tells Abby that he found his present hanging on his locker.  He tells her "it was subtle, very subtle".  The doctors have to stabilize Denise's pelvic fracture, so they leave Neela in the room with her holding the sheets tightly to keep the fracture stable.  Susan sends Pratt over to assist Jing-Mei.  She asks him if he is sure he can tear himself away from the new med. student.  Chuny walks in to get something from the room that Neela is in and Neela asks her if they are together, referring to Jing-Mei and Pratt.  Chuny tells her yes.  Neela's hands are beginning to cramp from holding the sheet up for the fracture stabilizing.  Chuny leaves the room to get some towel clips to assist her in holding the sheets.  Denise begins to tell Neela what happened during the accident.  She asks how Matthew and Katie are.  Neela struggles to hold the sheet.  Hannah is in the next room with Carter and Abby.  She goes into V-tach and Denise can see from the next room that something is going on.  Mrs. Johnson starts to crash and Neela begins to yell for some help.  Elizabeth walks in and sees that she is bleeding out.  Jing-Mei and Pratt come in to the room to see what is going on.  Jing-Mei points to Neela and says, "You, go pick up her labs".  "Right now?", Neela asks and Jing-Mei replies in a smart tone, "no, next Thursday".  Neela heads out of the room.  Neela searches her way to the elevator.  Carter and Abby continue to work on their young patient.  Neela gets up to the lab and finds Dr. Shine.  He has the lab work for Denise Johnson.  He tells her that it is an unusually smear.  Neela runs downstairs and tells the doctor's that Denise is anemic and saves her.  Pratt is impressed, but Jing-Mei isn't.

The phones are all messed up and Jerry and Frank are getting calls for all of the hospital instead of calls just for the ER.  Frank tells Jerry not to answer the phones anymore.  Gallant sees the new med. student and asks her, "Are you Indian"?  "Yeah, Punjabi", she replies.  Frank overhears and says "Perfect".  She tells him that her family moved to London when she was eleven.  She went to Yale for molecular biology and biophysics and Chicago for medical school.  Gallant takes a chart and follows her to assist.  Romano comes down to see what is going on and he has just returned to work.  Frank and Romano chit-chat.  Frank asks him how the arm is and Romano tells him that it is gone.  Romano asks Frank how his prostate is and Frank tells him that it is four times the size of Romano's head and he is up four times a night to pee.  Frank also comments about Romano's beard.  He wants someone to page Weaver for him.

Pratt goes in to where Abby and Carter are working and he asks Carter for a minute.  Carter tells Abby to call him if she needs him and she replies, "I won't".  Pratt tells Carter about an HIV patient that has returned and has sores in his mouth.  His condition has worsened.  Pratt asks Carter what all the with Abby was about and Carter can't answer him.  He tells Carter that Jing-Mei is upset with him too.  Carter asks him what Jing-Mei is upset about and he tells Carter that it was a misunderstanding.  Carter sees that the HIV patient isn't on Medicaid and that he is uninsured.  Pratt tells him that he referred the patient to ADAP.  ADAP is out of money and it will be at least two months before they can get him started on his med's if he is only HIV positive.  Carter and Pratt go in to check on Mr. Williams.  He has been HIV positive for two years.  He isn't on any medications.  Carter tells him that he has good news and bad news.  The infection that he has now qualifies the patient for Medicaid.  The patient has AIDS.  The bathrooms are under construction, so Susan and Abby go into the men's room, as they discuss Carter.  Gallant and Neela assist an older woman.  She tells Neela that she has beautiful skin, "What a lovely color".  She will need to go up to the O.R. and Gallant tells Neela to check her vitals every fifteen minutes and to get another patient chart.  Susan is upset to see that Romano is back.  Kerry and Romano argue about the condition of the ER.  Frank tells Carter that the guy from the foundation will be coming by with some papers.  Carter doesn't want him coming to his work.  Carter and Abby decide to get a cup of coffee later.  Mr. Williams stops Carter in the hall and tells him that he filled out the paperwork and went to the pharmacy, but can't get the medicine until he gets his card.  Carter will call the pharmacy.  Neela's patient tells her about her late husband and how they met.  Neela draws some blood as she talks.  She tells Mrs. Ferguson that she is all done and that she will be back soon.  The workers are going home and this upsets Kerry even more.  The phone's are ringing and nobody is answering them.  Neela decides to pick the phone up.  "Hello, Emergency Department.  I'm sorry I can't hear you very well", she replies.  She tells Frank that it is long distance for a Dr. Carter.  Frank tells her to take a message.  "Yeah can I take a message", she replies to the person on the other end of the phone.  Neela tells Frank that they say it is important, someone has died.  "Someone named Kovac, a Dr. Luka Kovac".  Jing-Mei, Chuny, Gallant, Frank and Jerry look at her in surprise.

Carter and Abby are on their way back from getting coffee.  He tells her about what Africa was like and how it was a life changing experience.  She asks him where Luka is.  "Is that what this is about", he asks her.  She replies, "No".  He tells her that Luka is fine, "He's more than fine".  She asks him what that means and he tells her nothing.  This aggravates her because she doesn't think that they talk anymore.  She didn't want him to go, but he went anyways.  She asks him if he even thought about her while he was gone and he tells her that he did.  She then wants to know what he thought about, but Carter pauses and doesn't know how to reply.  Abby walks away and Carter tells her to "Walk away, that is what you do best".  Abby storms back and tells Carter that he has big problems too.  As they are arguing Chuny comes out and tells Abby about Luka.  Carter calls to Africa to find out what happened.  Frank asks Kerry if he had family here.  She tells him no.  Neela asks Gallant who Luka was and he tells her that he was an Attending.  "Friend?", she asks him.  "No. not really", he replies.  He asks her how her elderly woman patient with the shunt is and she tells him that she is going to go and check on her.  "Surgery should be down soon", he tells her.  Neela doesn't find her in the room where she was, so she goes to locate her, but Pratt stops her and asks her if she is ready to present the patient in Curtain One.  She begins to tell him and Jing-Mei comes up.  She questions her on her presentation and she tells her that she needs to do a bowel prep on the patient to be safe.  Pratt tells Jing-Mei that she has been too rough on Neela.  "you were all over her and it is humiliating and embarrassing", Jing-Mei replies.   Malik comes out of a room and tells Pratt and Jing-Mei that there is an old lady bleeding out and they run in to see Gallant's elderly woman.  She is not bleeding and there is no pulse.  Neela goes in to see what is happening.  She tells Jing-Mei that she was suppose to be checking on her every fifteen minutes.  Gallant tells her that it was his responsibility.  She explains that she looked for her, but didn't see her.  She thought she had been taken up to surgery.  Malik tells Jing-Mei that he moved her into that room for more privacy and changed it on the board.

Carter is still on the phone trying to learn more about Luka.  Abby walks in and asks Abby if he knows Luka's father's first name.  Carter gathers some supplies and tells Abby that he is going to go and find him.  He left Luka at the clinic in Matenda and he should have stayed there with him.  Kerry sees him taking supplies and he tells her that he will inventory it and that she can bill him.

Pratt walks in to see Neela with her elderly patient.  She asks Pratt what a TWA means.  He wants to know where she heard that from?  She tells him that the old guy at the front desk with a flat head called her that when she arrived this morning.  "Third World Assassin", he tells her.  He tells her that he wants to show her something.  He takes her to the room where Denise Johnson is.  As they look at her Neela asks, "Was that today"?  "Thirty-two year old mother of two with a rare blood disorder.  Still alive tonight", Pratt replies as he points to Neela and she smiles.  Carter heads out of the hospital and Abby tells him to just stay, she doesn't want him to go.  "I can't leave him there", Carter replies as he walks away.  "Please just stay, I can't keep doing this", she yells, but he continues to walk away.

OUT OF AFRICA, WEARY CARTER RETURNS TO HOSTILE ABBY -- AND PARMINDER NAGRA ('BEND IT LIKE BECKHAM') JOINS CAST IN SEASON PREMIERE -- In the season premiere, a frazzled Dr. Carter (Noah Wyle) returns from his harrowing mission to the Congo only to be confronted by a hostile Abby (Maura Tierney) while an exotic new British medical student (Parminder Nagra, "Bend It Like Beckham") causes a stir when she unexpectedly helps a patient -- and angers Dr. Chen (Ming-Na) as Dr. Pratt (Mekhi Phifer) flirts with her. In addition: a concerned Dr. Pratt works the margins to help an HIV-infected patient obtain the necessary medical insurance; a woman and her young children are treated after their wrecked car is struck again by a large truck; construction workers add to the chaos of the emergency room; Dr. Romano (Paul McCrane) sports a new look -- and Carter gets an ominous call from Africa. Laura Innes, Alex Kingston, Sherry Stringfield, Sharif Atkins also star.

TV Guide Review

September 25, 2003: Now What?: The 10th season opens with County undergoing extensive renovations, Carter out of Africa but in trouble with Abby, and a new med student  (Parminder Nagra) coming between Chen and Pratt.  Meanwhile, a mother's hidden ailment causes a tragic car accident.
