A young lady is sitting in one of the ER exam rooms alone.  She gets up out of her bed as we hear her mother out in the hallway telling Chuny that her daughter does not need to see a psychiatrist.  The girl walks into the hallway unnoticed and then heads upstairs.  Carter is on the phone with Dr. Weisman about a patient of his that has been waiting.  Randi interrupts Mark and Elizabeth, as they are discussing a patient's care, to tell Dr. Greene that Dr. Romano has called and wants to see him up in his office to discuss the Fossen case.  Peter sees Mark and asks him if Cleo has been sick.  Dave and Malik announce that Dr. Weaver is on her way in to work.  Abby and Luka head outside to meet an ambulance that is bringing an MVA in.  They are discussing about renting an apartment, but Abby seems to be happy where she is already.  As they are discussing this, someone comes crashing down onto the canopy above the ambulance bay.  She tumbles over and it is the young lady that had previously wandered out of the ER exam room.  The remainder of the episode is seen through the eyes of the following four characters:

Weaver:  Kerry stops by a street vendor and buys a necklace.  She drives in to work and parks.  She has been listening to a talk show about homosexuals in the workplace and is hearing them talk badly about it.  She walks in to be greeted by both Dave and Malik.  She has been gone for 3 weeks and Malik and Dave try to act as if she has been missed.  Randi compliments her on her new hair color and about her new bracelet that she must have bought in Africa.  Kerry mentions that she has been to Kenya before and then walks into the lounge and pauses in front of the locker.  She hears a loud crash and looks out the window to see a young lady lying on the ground.  The patient is brought back in to the ER and Luka, Abby , Kerry and the others work on her.  Kerry finds out from them that she is already an ER patient and Chuny tells them that the girl is pregnant with her brother's baby.  Kerry wants to know why they left this patient alone and Haleh says that Psych has been short handed since Legaspi left.  Someone mentions that Kim got a job in San Francisco, and Abby asks Kerry if that is true.  Before Kerry can comment, she gets squirted by blood from the patient.  Peter has walked in and takes over, since this was his patient.  Luka tells Kerry to go and get cleaned up.  Kerry has washed up and comes across Jing-Mei in the hall with a patient.  Jing-Mei asks her about her trip and Kerry tells her that it was hot.  As Kerry walks away, she thanks her for the Chief Resident confirmation that she has received.  Kerry smiles and then hears a cry.  Luka has told the young lady's mother that she has died.  Kerry hears a disruption.  It is Jing-Mei's patient in a wheel chair.  He is arguing with a woman, who really is a man.  They had been dating, until it was revealed on a talk show that the lady is really a man waiting on one of his last operations to become a woman.  Deb tells Kerry that the man, Mr. Peterson, was in a fight on the talk show.  Kerry is offended by the name calling, and then the man grabs Kerry and tells the transsexual that this is the way it is suppose to be.  He points to himself and says, "MAN!", and the points to Kerry and says, "WOMAN!".  Luka and security come and calm the man down.  Kerry asks Luka if the staff has been talking about her while she has been away.  He tells her that people usually talk about their boss.  Before he can really answer, Dr. Anspaugh walks up and they go to the Derek Fossen M&M.  As Mark is presenting the case, Kerry's mind wanders to Kim and she can not focus on the topic at hand.  She is finally asked a question by Dr. Anspaugh, and she is able to fumble out a response.  Kerry sees Romano afterwards and wants to know who he has told.  He acts as if he does not know what she is referring to, but finally answers.  He assures her that he has not spoken with anyone about it and that her and her sexuality is confidential and not appropriate talk for the workplace.

Benton:  Peter is at his brother-in-law Walt's garage having his car worked on.  Joanie, Peter's niece, is taking care of the garage's finances and tells her uncle Peter that he should pay her father.  She tells him that the family is having some financial problems, because her mother has lost her job.  Peter is on his way out and she ask him to bring Cleo over sometime.  He tells her that they are on a break because Cleo is going through some things.  Back at the ER, Peter takes care of the talk show host.  He was bitten by one of the transsexuals that were on his show.  Peter sees Cleo and he asks her how she has been doing with her meds.  She tells him that she has been nauseas, but she is okay.  Peter assist with a 20 year old girl, Alice, who fell and broke her wrist on a talk show and was hit in the face and abdomen with a chair.  She had found out that her boyfriend was actually her half brother.  Alice's mother comes in and tells her that the producer made her tell her.  Alice tells her mother that she knew they were having sex.  Haleh takes the mother out of the room, seeing how upset Alice was getting.  Peter does an ultrasound to check on the girls abdomen and sees that she is pregnant.   Peter tells Alice that she is about 10 weeks pregnant.  Alice is even more upset and says that she was planning on keeping the baby.  "Why is this happening?", she asks.  Peter wants to get a CT of her head, so he leaves Alice alone in the room and also asks for a psych consult for her.

Peter then treats one of the transsexuals on the show. He/she has a head laceration.  Peter gets distracted when he sees Cleo across the hall vomiting.  He speaks with Mark and asks him if Cleo is having any trouble with her triple cocktail.  Mark tells him that he should speak with Cleo about that.  Peter's niece, Joanie, comes in and expresses concern that her mom and dad are going to split up.  He says that it is normal to be like that after losing a child.  He tells her to give it time.  He then gets a page and has to rush back to the ER.  His patient, Alice, has jumped from the hospital window.  Peter and the rest work on her, but she dies.  Luka leaves to tell the mother.  That night, Peter stops by and sees his sister Jackie.  She is cleaning out the garage.  He tells her that he is checking on her, he had heard that she had lost her job.  She just can not get over Jessie's death.  She can't quit thinking about all of the "last times" she will miss with her son.

Carter:  Carter is in a limo with a woman.  They are both dressed up and are on the way to a funeral ceremony.  The woman is his mother, Eleanor Carter.  Carter's grandfather has passed away.  He comforts his "Gamma" at the graveside service.

After the ceremony, Carter and his mother are on their way back to "Gamma's" house, when he receives a call.  He tells the driver that they will need to stop by County General Hospital for a moment.  He is checking in on his patient, Holly Evans, who is a little girl waiting for a hospital bed.  Abby is the one who had called Carter and she is looking after Holly.  Carter calls for a Dr. Weisman.  Abby jokingly says, "Nice suit...who died?" as Carter is waiting on the phone.  He tells her that his grandfather died.  She probably felt like she had stuck her foot in her mouth and offers to go with him to pay her respects, but he assures her that it was okay and that he would not subject her to that.  Carter and his mother arrive at the Carter estate.  They are greeted by his father, Jack.  John wants to check on his grandmother, but his dad assures him that it would be better for him to just mingle with the guest and to be a gracious host.  Carter looks at a photo of himself and probably his brother hanging on the wall.  An employee of the family tells Carter that his guest has arrived; it is Abby Lockhart.  They take a walk and sit out on the lawn near the bird bath.  Abby notices that it is broken and comments on how there will be dirty birds flying around.  Carter tells her how is grandfather dies while mowing the lawn and ran into the birdbath and broke it while having a fatal MI.  Abby and Carter snicker a little about it and what a funny way it was to die, but Abby still knows how Carter must really be hurting.  She felt bad that she is close to Carter and didn't even know about the death.  John's mother walks up and Carter introduces Abby to her.  Eleanor is a little rude and asks Carter to go and fetch his grandmother; it is time that she make an appearance.  As she walks away, Carter tells Abby that they don't handle death very well and that it had been a long time since they had to deal with death; not since his brother had died.  Abby leaves and Carter goes to search for "Gamma".

He finds his dad looking at the television and checking on his stocks.  Carter tells him that they should stay with Gamma for the night instead of a hotel.  He tells his son to leave it alone.  Carter says that he has left it alone for many years.  He tells his dad to get a backbone and to stand up to Eleanor.  He tells his father that Bobby is not coming back.  His father turns away and Carter starts to walk out.  He then tells Carter where he will probably find his grandmother.  He finds her in the garage among the many fine cars that they have collected.  She is sitting in one of the convertibles, trying to crank it.  Carter hops in and sees that she has a bottle that she was saving for their 60th Anniversary, that they had missed by 1 year.  John opens the bottle and they drink to his grandfather.  She gets the car to start and they drive off with the wind blowing through their hair.

Greene:  Mark gets an MRI and is apparently seeing his doctor for a checkup.  He asks the radiology tech to read the results for him, but he says that he will have to wait for the radiologist to read it to him.  Mark sees a patient, who is a woman who had taken her daughter on a talk show to reveal a big secret.  Luka tells Mark that he will be presenting the Fossen case in an M7M later on.  Mark sees that Cleo is throwing up in an exam room and he checks on her.  He tells her that she needs to focus on taking care of her liver by reducing the amount of her med doses.  Mark sees Adele Neuman and asks how her physical therapy is going.  She tells him that she has been able to stand a walk with some assistance, but she is still in a wheel chair.  Elizabeth and Mark work on a patient who has pneumonia and is a drunk.  She asks Mark how is doctor's visit went and he replies that he still has a brain and is okay.  He asks her how she is doing and she lets him know quickly that 6 weeks maternity leave was not long enough.  Elizabeth wants the patient to have surgery, but Mark thinks that he would be better off and safer without it, since he probably lives on the streets.  Mark is called away by Romano wanting to see him.  Mark goes to his office and he asks Mark all that he knows about the Fossen case.  Mark starts to tell, but they are interrupted by Brenda, his secretary.  She is scolded by Robert for interrupting, but she tells them that it is an emergency.  A woman has jumped out of the 4th floor hospital window and that she was an ER patient.  Mark gets to explain what happened about the Fossen case, but he neglects to tell the truthful story.  Mark presents the case at the M7M.  He is able to answer all of the questions without causing much concern.  Elizabeth is looking over the paperwork about the case and sees that the patient was conscious during the whole time he and Mark were in the elevator.  If Mark did what he said he did, it would have been brutal for the patient.  Dr. Zagerby says it was barbaric, but nobody seemed to be too concerned about a serial killer dieing.  Mark returns to the ER to find Mrs. Jenkins in the room with her daughter's body.  She tells Mark that everything she said on the talk show was all a lie.  Alice's boyfriend was not her half-brother.  The producers of the show paid her to say that.  She had always told Alice that bad things would happen if you told lies.  That night, Elizabeth comes home to find Mark holding his baby daughter, Ella.  Mark tries to tell Elizabeth the truth about what happened in the elevator with Fossen, but Elizabeth replies that she had heard enough about Fossen for the day.  Elizabeth kisses the two of them goodnight and then heads upstairs to bed.

SEASON PREMIERE OFFERS DIFFERENT TAKES AS GREENE CONFRONTS FATAL DECISION; MARY MCDONNELL AND MICHAEL GROSS GUEST-STAR -- The season premiere offers multiple points-of-view of the same events a la "Rashomon" as Dr. Greene (Anthony Edwards) ministers to a sobbing young woman (guest star Jessica Townsend) who was injured during a confrontational television talk show melee where she learned bitter family news -- but Greene has to face his own demons when a board of inquiry investigates the case of his serial killer patient. Meanwhile, a stoic Dr. Carter (Noah Wyle) must attend a family funeral which reunites him with his detached rich parents (guest stars Mary McDonnell, "Dances with Wolves" and Michael Gross, "Family Ties"). Dr. Benton (Eriq La Salle) keeps a watchful eye on the recovering Cleo (Michael Michele) while he deals with his grieving sister's (guest star Khandi Alexander) attempts to cope with her teenaged son's death. A nervous Dr. Weaver (Laura Innes) returns from her "African" sabbatical and warily waits to see how her colleagues react. Alex Kingston, Goran Visjnic, Maura Tierney, Paul McCrane, Erik Palladino and Ming-Na also star.

Jumping back and forth in time over a single day, the episode follows Carter as he deals with his chilly socialite parents (Michael Gross, Mary McDonnell) during his grandfather's funeral; Greene, who is hiding the role he played in a killer's death; and Weaver, who returns to work fearing she's been outed. Amid all of this, Cleo suffers the side effects of her HIV treatment; Benton's sister (Khandi Alexander) begins to unravel over the loss of her son; and the ER is filled with patients after a brawl erupts during a local talk show.
