It is Thanksgiving day and Carol gets up and goes in to work on her day off to show Lydia how to do some paper work.  Lydia has already left, so Carol heads back home.  She gets back on the train and her water breaks.  She gets off of the train and sits on a bench while waiting for the train that goes back to the hospital comes by.  Kovach gets off the train and sees Carol there.  Kovach helps her get back to the ER.  

  On the way, Carol gets tired and starts hurting, she passes out and Luka has to carry her the rest of the way.  David, Rachel, and Mark Greene go to Elizabeth's for Thanksgiving dinner.  Carter and Malucci are working.  Carter finds out that Dr. Dave went to med school in Granada.  Carol is dilated 10 centimeters and Kerry is going to have to deliver in the ER.  Carol does not like the idea.  Mark is her Lamaze coach and he gets paged.  He leaves his father and Rachel with Elizabeth.

The first baby is a girl and she names her Tess.  She is then moved to the OB and is cared for by an OB nurse named Abby Lockhart.  Dr. Janet Coburn is going to deliver the next baby.  Mark is there to help.  Back at Elizabeth's, Rachel is in the bathroom and will not come out.  Elizabeth finds out that Rachel got her first menstrual period.  She is only 10 years old and David is surprised.  They send him to the store to buy some sanitary napkins.  Carol is having a hard time delivering the next baby.  They will have to perform an emergency c-section because the baby's umbilical cord has come out first and is loosing oxygen.  She has another little girl and she names her Kate after Mark's mother's middle name.  David Greene slips and falls on the ice and hurts his head.  They have to go to the ER to get stitches.  Mark's dad really likes Elizabeth.

A HARROWING THANKSGIVING TO REMEMBER FOR PREGNANT HATHAWAY; MAURA TIERNEY ('NEWSRADIO') GUEST-STARS -- A snowy Thanksgiving turns chaotic for Hathaway (Julianna Margulies) when she suddenly goes into labor on the subway and fights just to get to the ER -- where the real harrowing battle begins to save her premature twins with the help of an obstetrics nurse (guest star Maura Tierney, "NewsRadio"). At the same time, Hathaway's Lamaze mate, Dr. Greene (Anthony Edwards), is unaware of her plight as he brings his precocious daughter (guest star Yvonne Zima) and his cantankerous visiting dad (guest star John Cullum) over to Dr. Corday's (Alex Kingston) for a traditionally dysfunctional holiday dinner. When Greene is notified of Hathaway's situation, he leaves poor Corday with his two relatives who each encounter a personal crisis. Dr. Malucci (Erik Palladino) impresses Dr. Carter (Noah Wyle) with his knowledge of a rare affliction. Laura Innes, Kellie Martin, Paul McCrane, Goran Visnjic and Michael Michele also star.

On an elevated-train platform, Hathaway goes into labor. Fortunately, Kovac happens by and helps her to the hospital, where she's determined not to have her babies in the ER. Her Lamaze coach, Dr. Greene, is needed, causing him to bow out of Thanksgiving dinner at Corday's condo. That leaves the British-born doctor playing awkward hostess to Greene's daughter and his cantankerous dad. Complications ensue — in both Corday's meal and Hathaway's delivery.
