Carol is really showing now.  She gets on the train and has to stand up.  A man asks someone who is sitting down to give up his seat, but he says that it is not his fault that the lady is pregnant.  When she gets off the train, she goes to a coffee house to get a warm milk.  A woman in line with her asks if she can feel her stomach and Carol tells her no.  The guy who didn't give up his seat is in the coffee house also.  Right after Carol steps out of the coffee house, a truck comes crashing right through.  Several people are injured and rushed to the ER.  Carol gets out of the ambulance covered in blood and Kerry asks her if she is okay.  She tells her that she is and that is not her blood.  Peter is working on his trauma fellowship now.  The guy who didn't give his seat up wants to speak to the manager.  He is in a lot of pain.  Carol comes over and he recognizes her and asks her who she is, Mark replies, "She's the manager".  Dr. Luka Kovach is a new doctor that is moonlighting a couple of times a week.  He is really good with the kids.  Lucy asks Kerry if she would be her mentor and Kerry agrees.  The nurses hear that Dr. Anspaugh is quitting his chief of staff position.  Mark and Kerry meet to find out what is going on.  Dr. Greene speaks out against Romano in a staff meeting and Kerry doesn't.  She later tells him that she knew he would end up with the position and she needed him on their side.  She ends up with the Chief of the ER position.  Cleo Finch is the new pediatric resident.  She runs 4 miles a day into work, Kerry and Lucy might just hate her for it.  The woman who wanted to touch Carol's belly in the coffee house dies.  A baby with AIDS is brought in and Jeanie has the baby tested for HIV without Cleo's permission.  Mark and Elizabeth are still going out with each other.  Carter tends to a woman who is his cousin's ex-wife.  She was in an auto accident, but she is okay.  Benton is late for his mediation appointment with Carla.  They are there to fight for custody of Reece and to keep him in the country.  Benton has moved in with his sister Jackie.  Reggie and Jeanie discuss fostering the little boy who has AIDS.  He asks her to marry him.  Kerry tells Carter that he can't continue living in her apartment and tells Lucy that she can't mentor her.  This is because she is the new Chief of the ER and it might cause some friction.  Carter meets with Elaine.  Mark's father calls to tell him that his mother has passed away.

NBC Review

REBECCCA DEMORNAY GUEST-STARS AS TRUCK CRASH YIELDS CRUSH OF PATIENTS -- An out-of-control truck barely misses a very pregnant Hathaway (Julianna Margulies) but cuts a wide swath of heavy casualties who flow into the ER, taxing the collective abilities of everyone on shift. Dr. Carter (Noah Wyle) unexpectedly treats the older and attractive ex-wife (guest star Rebecca DeMornay, "The Hand That Rocks the Cradle") of his cousin with unexpected repercussions. Drs. Greene (Anthony Edwards) and Weaver (Laura Innes) plot strategy when they hear rumors that the combative Dr. Romano (new series regular Paul McCrane) might be named as the new chief of staff. Dr. Benton (Eriq La Salle) must maneuver quickly during a mediation hearing if he is to prevent his ex-girlfriend Carla (guest star Lisa Nicole Carson, "Ally McBeal") from leaving the country with his infant son. Elsewhere, Luka Kovac (Goran Visnjic, "Welcome to Sarajevo"), a doctor from Eastern Europe who moonlights in the ER, and Dr. Cleo Finch (Michael Michele, "Homicide: Life on the Street"), a second-year resident in emergency medicine and pediatrics, both reveal a talent for handling hurting children. Gloria Reuben, Alex Kingston and Kellie Martin also star.
