Mark helps to get the kids off of the bus that got hit by a snow plow.  A girl named JJ is trapped with an open fracture.  A boy named Dennis is also trapped with what seems to be a spleen injury.  Carol speaks with Anspaugh about wanting to keep her clinic open.  She offers to quit the clinic and have some one else run it.  Doug comes to and calls 911.  Jeanie is badly hurt, she was not wearing a seat belt.  Carol tells Lynette that Anspaugh is going to let the clinic stay open, but she will not be able to continue running it.  She wants Lynette to run it, but she does not want to.  Carter speaks to Chuny about what she thought she saw him and Lucy doing in the exam room.  He tells her that it is against the rules for a resident to be fooling around with his student.  He does not want her to be telling people about it.  She tells him that he and Lucy make a better couple than him and Roxanne.  A crane comes in to lift the school bus back up, so that they can get the trapped kids out.  Mark stays on the bus with them as they lift it.  JJ and Dennis then get pulled out and sent to the ER.  The parents show up in the ER looking for and asking about their kids.  Randi tells them that they will need to calm down and give her their last name and the child's name.  Kerry tells Jerry to find Jeanie, she needs her help.  Jerry tells her that nobody knows where she is.  Mobalage’s wife, Mrs. Ekabo, comes in and wants to speak to Mark, but he is really too busy to speak with her.  He has her wait in chairs until a better time.  Doug and Jeanie arrive by ambulance and she is conscious.  Kerry tells Doug to step back and not to help with anyone.  He is not working in the ER tonight.  Jeanie has a concussion and Peter runs some blood work on her.  Kerry tells Carol to take care of Doug, he will need sutures.  Doug tells Carol that he is going to resign.  He tells her that it is time for him to go.  He will take a job offer in Portland and move to Seattle.  He wants Carol to go with him.  She tells him that she grew up in Chicago and that her family, friends and job is there.  She leaves the room.  Mark has Carol talk to Mobalage’s wife.  She is concerned about his surgery that is scheduled for his erectile dysfunction.  Kerry tells Peter that Jeanie is in bad shape, but it is not due to the accident.  Mobalage’s wife tells Carol, that after the soldiers had come to get Mobalage, ten of the soldiers took turns raping her all through the night and up to the next day.  She never told him about it and she does not think that she will ever be able to have relations with him after something like that.  Carol tells Mark about it and he agrees that they both need to be counseled.  Kerry goes to speak with Jeanie.  She tells her that she may have Hepatitis C.  This is why she has not been feeling well lately.  She tells Kerry that she has worried all this time about HIV and hear she ends up with Hepatitis C.  Carol tells Mark about Doug wanting to quit his job and move to Seattle.  He tells her that maybe he should.  He loves Doug like a brother, but he can not take much more.  Carter waits for Lucy to talk with her.  He wants things to go back to the way they were.  He can not continue with what they started in the exam room.  She agrees that they are not compatible.  Mobalage’s wife is brought in by paramedics.  She has been stabbed by him.  When the police came he jumped out the window and ran off.  Nobody does not know where he is.  She is in bad shape and Carol is tending to her.  She sees a shadow in another exam room and goes to investigate it.  It is Mobalage and he is covered in blood and has cut his leg.  He is worried about his wife.  Kerry comes in and Carol tells her to go get Mark and call security.  When they come in to the room he drops to his knees and starts crying.  Carol finds Doug with Jeanie.  They leave her room and they talk.  He tells her that he has talked with Anspaugh and has quit.  The cops are not going to prosecute him over Ricky's death.  He knows that she will not go with him.  She tells him that she wants him to stay here, she does not want to wake up up alone tomorrow.  Mark and Doug sit and talk out in the cold by the river.

CARTER'S FIRST SAVE - Carter's first full day finds him saving a patient from cardiac arrest, his first such save. Later, he treats a coworkers rash and winds up going home with her. Meanwhile, the ER is inundated with both a tour group and a wedding party suffering from food poisoning from the same German restaurant. When Dr. Greene helps his wife, Jennifer (Christine Harnos), "celebrate" passing the bar, they get caught having sex in the ER bathroom. Dr. Ross visits a recovering Nurse Hathaway (JULIANA MARGULIES).
