Pilot -Season 6 Episode
134 "Such Sweet Sorrow"
The ER's Head Nurse, we meet Carol in the pilot episode shortly before she tries to commit suicide (Carol was originally intended to die).
Season 1

Carol returns to work six weeks later, we are told that she is going out with
Tag, an
Orthopedic Surgeon. Doug does not like this as he blames himself for her attempted suicide, he still has a flame for Carol.
Episode 16 - Carol is told that she cannot adopt Tatiana, a young HIV positive polish girl that was left in the ER as she previously attempted to commit suicide.
Doug and Carol still work side-by-side as doctor and nurse, even though they
had a failed relationship.
At the end of Season 1 Tag and Carol are due to be married. Tag realizes that Carol does not love him the way he loves her and the marriage is called off.
Season 2
Carol is in a relationship with Shep a paramedic, Carol buys a new house (which is a real lemon), and her life seems to be going quite well.
Shep then starts having problems and when Raul his partner dies Shep is devastated. Things start going wrong between Shep and Carol and they soon split.
Carol is putting a lot of her energy in being a good nurse and making a
difference.
Season 3
In Episode 049 "Let The Games Begin",
Carol has her car repossessed because she is three payments behind.
Times are tough because of the cut back in shifts. She wants to sell
her home, but the real estate rep tells her nobody would buy it.
Carol has a career crisis and takes the Pre-Med exams, passing in the 85th percentile, but Carol decides to remain a nurse.
Season 4
Doug and Carol eventually get back together and things seem to be going very well, minus a few hitches. Doug even proposes to Carol, to which Carol says yes.
Season 5
Carol
is getting stronger by the season. She is still sticking up for what
she believes and going after what she wants.
Doug and Carol are still together and even start trying for a baby.
Doug resigns following the death of Ricky (terminally ill boy with
ALD) after Doug gave his mother Joi the code to the Patient Controlled Analgesia machine (possibly allowing Ricky to receive an overdose of Morphine causing respiratory depression and death).
Doug resigns and leaves Chicago. Carol then finds out she is pregnant.
She is still struggling with the fact that Doug is gone and is wondering how
she is going to tell him about the baby.
Season 6
Carol is now heavily pregnant.
She seems to be getting friendly with Dr. Luka Kovac.
Episode 121 "Great Expectations"
- Carol has her baby daughters and Mark is there to coach her and give her
support. Tess is delivered first by Kerry Weaver in the ER. Carol is
sent up to OB for the second one. Dr. Janet Colburn will have to
perform a Caesarean Section due to Fetal distress. Since Mark is there to
help, Carol names the baby after his mother's middle name, Kate.
Episode 134 "Such Sweet Sorrow" - Carol goes to visit Doug in
Seattle after figuring out that Doug is everything to her and her soul mate.
